The guy in the picture got that tattoo while he was drunk. It was supposed to be a Charmander, but it wound up being that freakish looking thing instead. One of his friends posted his tattoo here to reddit, and it got really popular. There have been many artistic recreations of the tattoo and even an iPhone/Android game based on it.
