Yes, it exists! Give ScoutLog a try, it's an Android app that lets you save places, along with photos and notes, and tags to keep them organized. You can sync your places and photos to a Google Drive account, and then use the Chrome extension (still in beta) to edit them on the desktop.
You can also import any existing starred places you have from Google Maps or existing photos on your phone that have GPS EXIF data.

There are also Pokemon specific mapping tools but I prefer the generic ones. They're more likely to not infringe some copyright stuff and if I invest time into creating my own map the app should be available for a long time.

It stores all the data on your phone so you don't need to connect to the internet except for the Google Maps tiles data (which we're not allowed to save within the app).

In the meantime, it's super easy to save your current location, it just takes 2 taps: 1 to open the app, and 1 to add a new location (the app automatically sets your current lat/lon for the location).

You should check out ScoutLog, which can do everything you're asking for except for adding labels directly to the map. But, you can add places and then give them titles so that when you tap on the pin in the map, the title appears in a popup above the map pin.
If you want to spend the time, you can also download a bunch of icons to represent different places like parking garage, library, food/cafeteria because ScoutLog lets you attach a custom icon to a tag, and that icon will be used as the map pin if you attach it to a place. (Or you can assign colors for tags as well.)

This isn't exactly what you're asking for but it may suit your needs, depending on what they are.
ScoutLog is a location bookmarking tool that has support for saving photos. So, if you're at a location, you can open ScoutLog, create a new place and add one or more photos for it. The photos themselves won't appear on the map, but the place itself will appear as a pin. If you find the app useful for your needs, you can also import existing geotagged photos by selecting them in an app like Google Photos and sharing the collection to ScoutLog.

There was a nice service called Pin Drop, but they're gone because they didn't got a new funding and after that Apple hired them :/ I really wish Google would have bought them because their product was way more useful for the average user than Google My Maps.

If you're only looking for the ability to take photos and put them into folders, then you probably want a gallery app, maybe something like QuickPic.

If you have that many saved places, you should try a true location bookmarking app, like ScoutLog. Save places along with photos and notes, and tags to keep them organized. You can even import your saved places from Google Maps:
Go to https://www.google.com/bookmarks/ and click Export bookmarks. It should download a GoogleBookmarks.html file.
Transfer this file to your phone, and then share it to the ScoutLog app. For example, to do this with Google Drive: Upload on your computer, access the Drive app on your device, then tap the menu button for the file, then "Send file" or "Send a copy", then choose ScoutLog.
You can only create/import up to 15 places in the free version, but if you buy the paid version and you have problems importing your 400 places, I would work with you to get them resolved.

All the data is stored on your device so you don't need internet to access them, and you can share the location to another app for navigation. It also supports syncing the data across multiple Android devices and the PC via Google Drive.

Google Maps does indeed let you star places, but keep in mind if you create a lot of stars, some of them might disappear on you. This has been a problem for a couple years.

As far as I know, Google Maps starred places is a pretty limited feature. I'm working on a location bookmarking app for Android called <strong>ScoutLog</strong> which lets you save places along with photos and notes, and categorize them with tags.
If you try the app out and like it, I'm working on a good way to import starred places from Google Maps, but that feature probably won't be out for another month or two. If you want to import starred places into the app now, you can do it manually and I can walk you through the steps.

I use ScoutLog for this purpose, you can open it, snap a photo, and it automatically saves your location and you can do tags and notes or add more photos. It's pretty neat. Also only Android though.
Ooh! I already wrote a review for this for r/androidapps a few weeks ago but I love this app! It's ScoutLog, it's a GPS enabled location bookmarking app, and it filled a need I didn't really know I had.
Essentially it's just a location notebook. You can open it where you are, save your location on a Google-Maps-based map, save keyword tags or add a note, and save photos of that location. It syncs across devices and has a Chrome extension. It can also pull GPS coordinates from your photograph's EXIF data.

I guess it's aimed at photographers so they can remember good locations to shoot pictures, but I see it fulfilling two uses for me:
Google actually had an app like this, "My Maps," but it appears to be dead. Google Maps itself is an option, but seems geared less around private notes and memories and more towards publishing your photos and reviews for other users. I could just use OneNote or Evernote for this, right? But they don't have GPS and a map neatly tied right into it, and the notes would get lost in the shuffle.
I doubt this will win the contest because it's a niche app, but I just hope more people realize how cool it is and the app continues to be used.
I recommend at least giving it a whirl for a couple weeks. When you're somewhere interesting, open a log and snap a photo!
