Nomie 2 lets you track pretty much anything. Also supports Dropbox sync and .csv export, and CouchDB integration.
Only thing I don't think it can do is notify/prompt you to track something, but I could be wrong.
Have you looked through the activity log for a particular task? I have a whole project of things that I want to do daily, like practicing Portuguese or taking vitamins. The activity log shows when I marked the task vs when I rescheduled it. Pretty quick to punch these up in a spreadsheet of you want it quantifiable.
Otherwise I would recommend nomie. Not in active development anymore but a great tracker of literally anything, with all kinds of useful graphs. I used to track everything from how often I drink coffee to how much time I spend reading.
It can track anything you want it to, and if it doesn't have that existing tracker you can create your own.
I use it daily it's so helpful. I track my moods and many physical symptoms that I have with my mental and medical health. There's so many ways to track things as well.
I've been using Nomie Pro for about two and a half months and I love it. I can track my moods, and it has options on how you like to track them. You can create your own tracker or use some of them from the store. You can even have it prompt a note after you enter your information, which helps me think about my feelings a bit more. I can work out what's making me anxious or whatever I'm feeling.
I do like Daylio as well, but I think Nomie has more options and it's all free. It has dropbox sync and it gives you a chart of your trackers as well. Great app, I use it every day.
I just have a reminder every day at 10pm that says: "Try to get some reading in." It's in Todoist so I can technically pull stats on how often I complete the task versus rescheduling or postponing it...
That's usually enough of a guilt trip.
Alternatively try an app like Nomie to track how much reading you do when, then draw super specific stats on your habits. Works for other stuff too, like how often you drink coffee or whatever...
I can recommend Nomie. You can customize it however you like, and compare different trackers with each other (for example, mood vs. exercise vs. sleep). OP (and everyone else that is interested): try Nomie Pro if you have Android. You simply create any sort of counter you want and gather statistics right away about anything. Simple, cool app and idea.

There's no all in one app that I know of, but there are some definite up and comers out there.
Otherwise there are lots of great triggers that use integrations between the plethora of specific-purpose tracking apps out there over on IFTTT. Any app with an API is a potential integration. Some programmer just has to code the integration first!
