A decent open source document scanner is Open Note Scanner.
Open Note Scanner perhaps? Its score isn't very good but it seems to be the only notable one. It's also available on F-Droid
If you're looking to preserve it in the best quality possible I'd recommend using an actual scanner. Either see if a friend has one, or perhaps try your local library. You might also try a FedEx Office location too.
Of course, you can find photo/document scanner apps for your phone, which might be better than just taking a normal picture of it. I use Open Note Scanner on Android for document scanning, though I've never tried it for photos so I have no idea how it'd work for that. There are plenty of other apps doing the same thing though; there very well could be one better optimized for photo scanning.
