Its created by guidebook but a separate app
EDIT: After a reinstall of the base Guidebook app I can now download the PAX East 2015 guide.
Just checked it, its made by Guidebook, it's actually the same app except its only for PAX. I assume it was made so they could more easily direct people to it.

Essentially there are QR codes hidden all around the expo, when you find one (if you have the official PAX East app) you can scan it with your phone. Once you have scanned all the QR codes you bring the picture that has been put together (each QR code represents one piece of a picture) up to the PAX staff and they'll give you an exclusive prize!
