I don't have any suggestions on an app that can teach you the chord your playing, but there are plenty of guitar Tuner apps that can tell you what note you are currently playing and if you are in tune.
There may be better ones out there but I have been using "Pro Guitar Tuner" as my goto for quick tuning using my phone. It will tell you what note you are playing, whatever it may be. See link below if you are an android user. I'm assuming there is an apple equivalent.

My computer is freezing up. Probably Mobo is having problems. About 2 generations behind on Haswell and Ivy-Bridge on my intel chipset, I'm still using an I-7 2600k sandy bridge http://i.imgur.com/DrFlbrL.png. Also, broadwell and skylake.

5:30 continued TL;DR notes about learning http://i.imgur.com/tcgXYNR.jpg: I basically almost got a good "D" chord sound down, but it sounds off still. Not sure why. I used https://play.google.com/store/apps/details?id=com.jrinnovation.proguitartuner&hl=en to ensure my strings were in-tune, and used it to "listen in" and give me feedback on how each string plucked sounded, with different frets/chords on. I learned how to tune my guitar today. Had to cut my nails again. I have much stronger calluses now to hold down strings. Started getting into justinguitar.com WATERFALL lessons and using that as my main metric for learning.
All in all, not a bad learning experience. Those guitar / finger strength trainers made a giant difference in how well my muscle memory on each finger were. I try different combinations of fingers (e.g. Index+Pinkie, Indexie +Ring, Ring+Middle) to get used to that "muscle memory" sort of feeling when playing chords on a guitar
