It's not perfect but it does help! It was definitely worth the $50 or whatever I paid for it.

I’m a natural left side sleeper so I thought pregnancy sleep was going to be easy!
Right at ~22 weeks I started getting pain between my shoulders and up to my neck and absolutely cannot sleep through the night on my side.
I ended up propped on the couch with a heating pad on my shoulder blades by 3-4am nightly...I dumped my c-shaped pillow for a mini wedge and body pillow which didn’t help.
My Aetna assigned nurse recommended a recliner so I figured a big wedge was the closest thing without buying an actual recliner (we don’t own one ).
So i got a big giant wedge pillow which seems to be helping.
I am not too worried about the back sleeping / baby resting on the main vein... I feel like I just turn my hip a little and the baby is off center enough, but that may not work for you if you’re having hip pain too. I do recommend the belly band! That has completely eliminated hip pain for me.
I also have noticed my carpal tunnel acting up and got some basic compression gloves to help with that and it seems to help so far (about 2 weeks since I started using it ). Hope you get some sleep!
I can relate to a lot of your issues. I use a medcline shoulder pillow.
It solves four issues for me 1)shoulder pain is gone since my shoulder isn't mashed into the bed, 2) neck pain as my neck is level, 3) Acid reflux from being slightly elevated, 4) sleep apnea since being elevated opens my airways. Ymmv, but might be worth a try.

You can use the end of the pillowcase as an eye mask too.
I wake up with perfectly smooth hair and no smush lines on my face. I'm not creating any new neck or chest wrinkles because I'm sleeping on my back. It's been great for my posture as well. Cannot recommend enough!!!

I cover it with a satin pillowcase that I can swap out and wash. This travel pillow is, for me, the HG of back sleeping.
As part of the initial transition I used a weighted blanket and heating pad placed on my tummy, to simulate the warmth and pressure of sleeping on my stomach or in the fetal position. The weighted blanket I got was like $70 on Amazon but you can get them cheaper. I only needed those for like a week or two anyway.
The other critical thing has been an eye covering — a satin-lined hat pulled down over my eyes, the end of the satin pillowcase on my travel pillow, or a satin eye mask. Just be careful to only place whatever it is gently over your eyes and nothing tight or strapped to your face, bc it can mess up your eyes, which I learned the hard way, but fortunately before I'd done any permanent damage.
For my neck, chest and shoulders, I used to apply snail mucin power essence on wet skin and then tret, or snail mucin then triple oat serum and then tret. Lately I've been applying Stratia Rewind to wet skin, then Neutrogena Hydro Boost HA, then tret, then snail mucin, then oat gel moisturizer, and I've noticed it doesn't feel as thin as it did before. So I'll probably stick with this routine unless something changes. My 0.025% gel is almost gone and then I'll switch to the 0.025% cream I have on deck, and my routine might have to change then.

Genuinely one of the greatest purchases I have ever made. I cover it in a satin pillowcase and wear a silk sleep mask. At first I used a weighted blanket and sometimes a heating pad on my tummy to simulate stomach sleeping. Now I'm fine with just the pillow and eye mask. The other night I actually tried sleeping on my stomach and couldn't do it, it was giving me a headache, had to switch to my back which is INSANE after being a stomach sleeper for almost 4 decades!
Idk if it's making a difference in my skin but I can easily map the deepest wrinkles on my face neck and chest to smushing my face into a pillow and contorting my body for 8 hours straight every night. So I feel pretty confident this will at least slow those things down.

It's not expensive. I would buy it and try it. It will show her how much you wish you could cuddle. Put a bow on it and leave it on the bed. Make a nice dinner and do the dishes.
Then, during dinner, tell her you reeeeally want to try cuddling. But you're afraid that if it still hurts she will be hurt or disappointed. Tell her that her feelings mean everything to you and cuddling is the best part of your night. But then once the pain sets in it's hard to feel loving or anything else but pain. Ask her if, IF the pillow doesn't work, she can work on maybe holding hands? Tell her you don't want to come to any definitive conclusions and she doesn't have to decide now. But ask her to really try to think about how it might feel to be in your position. Ask her what she would want you to do if roles were reversed. Give her hugs before and after this conversation. Be very reassuring.
Think of it like a love battery. She needs to get that battery charged. So tell her you want to make up for it in other ways. See if you can come up with some fun, sweet ways to give her affection. Talk about it without her having to ask. If she knows you care and think about it when she's not around, she will feel soooo much better.
Late reply but what're you doing for vestibular therapy? BTW right there with you and your doctor and inconclusive test circles/ dizzy symptoms/ chronic forward neck posture/ straight neck x-ray. Can't buy a denneroll outright as only Chiropractors are able to, but my Blair Chiro recommended this item which is comparable, just not as aggressive. I just started using it 3 times a day and do get relief while I'm on it. So think it's just one of those things that will take a while to fix.
Hopefully things have gotten easier for you now, but thought I'd share in case it's useful for you or future folks -- I used this folding ramp pillow when I slept/laid in bed. Looking at the stats, it only has a height of 6.5" when unfolded, but it felt a lot higher than that. I usually put another pillow or two on top of it during the day (I slept with it as-is at night). Sometimes I left it folded in the box configuration to get more height -- it's tricky to get a full 12" above your heart, but I was really scared of clotting! During the day, I laid on my (non-sectional) couch and put operated leg up on top of the back cushions.
The biggest painful part of my surgery was enduring the neck and back pain the first few days from being in the same position constantly-- I used a heating pad and alternated between an Icy Hot roller and a CBD roller all day long. Once I was 1-2 weeks out and was able to sleep on my side and more easily change sides during the night, I was a lot more comfortable.

Wedge pillow is against the wall, standing up. Body pillow is oriented with the bend under my knees and the “arms” on either side of me. And then one of my regular bed pillows is against the wedge. I sort of slightly angle my hip to the side to lean against one arm of the body pillow.
Tonight, I’m going to try body pillow only with a regular bed pillow, with the u-bend at my head. I’m quickly getting tired of sleeping sitting up.

If you roll on your side, the pillow legs stabilize your head under your chin and at at your forehead, without touching your cheeks or eye area.
