Supplements like inositol, berberine and NAC can be helpful or meds like metformin.
If you search fatigue in the search box you will get a lot of hits.
Also, do you wake during the night or wake up with a dry mouth? If so, NAC or a cpap can be helpful re: apnea, a magnesuim supplement like Natural Calm and Breathe Right strips can help too. Mouth breathing at night or poor quality sleep spike cortiosl, which also increases insulin resistance.. Keeping cortisol and insulin both low is the key to managing PCOS symptoms.

For some people, longer fasts than AD can be counter productive, the keys to managing PCOS are keeping insulin AND cortisol low. Cortisol can be released as a counter-regulatory hormone on longer fasts. Experiment and see which group you fall into. One of the benefits of ADF for me was the consistency, I did not do longer fasts as consistently but I have done them without any issue. My symptoms pretty much reversed, so it is defo worth a try.

Doctors are not much help with metabolic issues and happily the tools to change it are in our hands. They do not get trained on this stuff at all. Anything up to prediabetes gets dismissed but that is the time to make changes like you are doing.

Fasting and keto are well known to reverse PCOS fully and restore fertility.
Usually it has to do with increasing insulin resistance or increasing cortisol or a combo, it is as though we each have a tipping point and after that gain weight very easily. But, you can walk it back. This article explains it

Actual medical doctors, Dr. Jason Fung and Dr. Eric Westman have decades of experience treating women with PCOS. They both speak of IR as being reversible and of LC & IF as being tools that promote metabolic healing.
Based on their training and extensive clinical experience, the symptoms that qualify for a PCOS diagnosis ARE reversible, without those symptoms, you may no longer qualify for a PCOS or diabetes diagnosis. It does not mean that if you start eating the SAD again it might not make you sick again, but that is a different issue. Dr. Ben Bikman's book, Why We Get Sick also delves into th issue of metabolic healing.
Both of these books are excellent for metabolic healing, as are any interviews with or articles by any of the 4 authors.

You really want to save your thyroid. These two books give hope.
She recommends Low Dose Naltrexone (LDN) And Myo-Inositol to help lower antibodies. These two have worked for me.
Other things can cause thyroid issues and antibodies to rise such as:
Mold, Environmental chemicals, household chemicals, body care products, low lying infections, heavy metals, B12 deficiency, anemia and more.
These are outlined in the books and you can order testing through her or she will tell you where to get them.
It sounds a little daunting but doing the research and finding the right help will improve the quality of your life.

I recommend this book, Dr. Fung and The Fasting Method have worked with women with PCOS for many years. In fact, co-author, Dr. Nadia reversed her own PCOS symptoms, as did TFM co-founder Megan Ramos. We are all different but if interested, try it and see. The kindle version of the book is very affordable and it has some good recipes.

For me, really focusing meals on 30g of protein and healthy fat (avocado, olives, etc) made me full so sugar and starch were not as appealing. Supplements and some IF helped with hunger and cravings, as my IR was reduced cravings kind of faded and my PCOS symptoms got way better. Good luck!
For me, layering in inositol and some intermittent fasting really helped lower the insulin resistance driving weight gain. With PCOS we have more insulin and clear it more slowly so spacing out eating gives it time to fall and stay low. I started with cutting out snacks and eating dinner early, then not eating until breakfast, so 14 hours or so. This article and book were really helpufl to me. Good luck!

I use recipes in this book a lot and found this book super helpful, so are vids by the authors. The great thing about fasting is that is works with any eating style and it is very effective for PCOS and insulin resistance.
I had two thinned out spots. I go after thick, wirey hairs that were dispersed throughout my head though. I did this YouTube hair regrowth recipe when I first stopped pulling and it did seem to help! Also, I am currently following the hormone balancing nutrition plan in this book and have been since November and I’m seeing a lot of improvement on hair texture and regrowth (apparently my thick, wirey hairs indicated a thyroid imbalance this whole time!)
This! Yes! I have most definitely noticed a complete change. I didn’t pull before birth control and I did after about a year of being on it. I’m currently working on getting off of it in two months, I’ve been on it for 12 years and I think it’s ruined so much of my mind and body.
Anxiety skyrocketed—also due to life circumstances—depression and suicide ideation also skyrocketed, I began having issues with allergies and general inflammation that I never had before, have been experiencing issues with my gut, liver, adrenals and thyroid the past two years. And now it appears I have a thyroid autoimmune disorder now, thanks to BCP (women are 300% more likely to develop an autoimmune disease on birth control pills)
If I were you I’d stay away from them!!!! There’s SO many other options!
I highly recommend reading Beyond the Pill before you commit to really stay on them. It has helped me SO much, I can’t imagine the difference it would have made if I read this 12 years ago instead of beginning BCP
I’m still unraveling the connection between trich and BCPs, but it’s VERY real. Your body has a delicate balance of estrogen and progesterone that changes throughout the cycle and these hormones, along with several others that are interdependent to estrogen and progesterone, help us with anxiety. When that is overtaken by the pill the impacts can wreck havoc throughout your system

This book re: PCOS and fasting is great, the authors also have a YT talk. You may also want to check out r/intermittentfasting and defo read the advice on electrolytes in the sidebar at r/fasting, no commercial products needed, they have excellent info on what is needed and quantitates.
I'd skip Audible, there are graphs and recipes that you will miss. My library had the book, I later picked up a copy to have the recipes. The Obesity Code by Fung is also excellent, and his team does The Fasting Method podcast, both hosts had PCOS and other insulin resistance linked conditions put into remission with diet changes and fasting.

Give Wheat Belly a read. I don't pretend to be an expert but I agree there's a distinct correlation between the "what is healthier" push in the 90s and how wheat breaks down into glucose (aka sugar).
I resisted that one and the heavy cream in coffee in fasting window for a long time despite listening to loads of Megan Ramos podcasts where she says fasting clean gives the best results. Alas, having worked with thousands of clients at The Fasting Method, turns out she was right.
Once I switched I also saw better results. She also really advises to focus on therapeutic fasting and get to weight and health goals in a finite time period. I wasted SO much time doing lazy LC with frequent cheats, loads of flavored drinks and cream in coffee in fasting window, etc. I felt both deprived and frustrated that I was not making much progress. Once I really dialed it in the changes were significant, my hair even grew back. Ramos talks about women losing more and faster the as they get more established in fasting, that was also my experience, as IR decreased, weight shifted more easily. Men tend to lose more at first, then slower, women kind of the opposite pattern.

OP its not unusual for women to have strong side effects with birth control, I recently found this book and its at the top of my to read list after years of hellish experiences with different pills and an IUD. Getting the science may help you plan out your discussion and feel empowered to stand your ground about your body and health. I recently developed spider veins on my thighs/cheeks- this happened while I was on Nexplannon. It got sooo bad especially when I was weightlifting consistently. Like to a point where I don't feel comfortable putting on a bathing suit. I got my implant taken out in May (I had it for a year) and the veins seem to be getting worse? I'm not overweight, I'm active, and I just learned that Nexplannon, like most birth control pills and implants, can cause this. Low blood pressure is a thing (and blood clots) and it's really fucked up because I'm only 24 and I shouldn't have to worry about this. They're not harmful, but if your family has a history of blood clots you need to tell you gyno asap
If you're considering other birth control methods- I highly recommend Beyond the Pill to just learn more about what birth control does to your body! I got off nexplannon for a number of reasons but I had no idea spider veins would be one of them.
There are options out there, it's just a matter of informed consent! Currently using the fertility awareness method and it's changed my life for the better. Hope this helps <3
