These are my absolute favorite. They're about $10 a pair, and they support the boys like nothing else. I'm typically about 5 soft but can vary slightly due to temperature. The trunks are the only ones you should buy. Trust me. The briefs are too small and the pouch is too tight, and the longer boxer briefs have too much material so you can feel it riding up. And if all else fails, shower well before trying them on for the first time, and send them back if you don't like them. I wear them hiking and day to day stuff and they never disappoint.

A few points:
First, I don't see any underwear like the ones I'm talking about from David Archy (https://www.amazon.com/gp/product/B00DYNPG4K/ref=oh_aui_detailpage_o08_s00?ie=UTF8&psc=1)
Second, DA does use Micro Modal on many of their products, including the ones I linked to.
Third, I can't pay $45 for one pair of underwear. I'd much rather get the 4 pack of DA's for $22.99 when it's on sale at Amazon.
I just recently backed T-Bô on Kickstarter, which is supposed to be the answer to all underwear problems ever. Mine should be shipping in the next couple of weeks. I'll let you know if they live up to the hype.
That said, I recently got these off Amazon, and while I've only worn them a few times, I don't hate them yet. Due to thicker thighs, I'm not a fan of actual boxer briefs, so they might not be your bag.

I also started taking horse chestnut and within 24hrs found relief from swelling and pain. Do your own research and determine if it's something that might be right for you. It worked for me and I honestly don't give a damn if there's a peer-reviewed study or not. I was literally laying in bed for the last 20 days and only getting up if I absolutely had to. I would sit in the bathtub with the water as cold as it could be just for pain relief. I know this subreddit doesn't like natural remedies and I am not trying to say that it's a solution over surgery. I don't think that horse chestnut is a cure. However, you may find that it provides relief from the pain. If you want to look more into it, there are studies regarding its efficacy in treating varicose veins. Embolization/surgery are excellent treatment options and they should not be ignored.

You should stop masturbating while this is a problem. However, if you do end up masturbating make sure to lay down flat afterwards with your groin elevated. This way the blood can naturally flow back out of your veins.
Amazon has David Archy boxer briefs and briefs that do exactly this. Far less expensive than most of those already mentioned. Under $30 for a 4 pack. I use them and recommend.

In terms of mens underwear that are boxer brief in style with seperate pouches for balls and penis check our david archy and sepertec on amazon. There are many other brands but these are good quality, well reviewed, and usually up on a daily deal every so often if you are patient. I wear a HTv2 or a happy go metal cage and have the below on currently and they work. My package underwear is also great though not available on amazon to the best of my knowledge. Not every brand and style will work equally for all, like finding a cage that works.

Absolutely my favorite underwear! Wore boxer briefs for years, but these separate your man parts, really comfortable, no sticking, good airflow.
personally I like calvin klein boxer briefs. But if you're on a budget just get hanes. Also be mindful of the length. If you wear fitted pants the boxer briefs with roll up. That was the problem with boxers for me, all the bunched up fabric whenever i pulled my pants up.
