Aside from that ballast being old-as-fuck (2003 manufacture date...) as long as it can actually light a bulb, it will work for 1000W HPS bulbs (and ONLY 1000W HPS)
As for the 'missing' cord; you are not missing it. Ballasts do not typically come with one. That cord is provided by the fixture you purchase to house your bulb (typically a hood like this)
You CAN buy bare Mogul Sockets, but ideally you are using a hood to house your bulb
