My girlfriend has a circulation problem that causes painfully cold toes even and uses these 1.5mm neoprene toe-warmer things (there are lots of brands and I'm sure you can get them somewhere other than Amazon if you prefer). For hands, chemical handwarmers + mittens is a very effective combo. Alternatively you could try electric mittens and bring a charger battery -- I have no idea how viable that is, but it seems potentially doable for shorter trips (esp since the heating element wouldn't be turned on except during the very coldest parts of the trip). In either case a pair of light fleece or merino gloves would be good to have for less cold stretches and as liners. (Neither mitten option is exactly a UL solution, but avoiding painfully cold hands is a major priority! Happy hands and feet, happy hiker!)
