I am very tired for it has been an eventful weekend so maybe I am not processing everything you mentioned or maybe I missed some stuff. But here is me trying to help.
> But I also have dermatillomania triggered by the Adderall so it's sort of a toss-up on the physiological benefits bit.
Any type of adhd stimulant or wellbutrin can cause this, that said it seems to be more common with amphetamine products. Adding intuniv (another adhd med a nonstimulant) may help you exercise the self control needed to stop the skin picking.
This has to do with how these meds work with different brain regions. Motor tics including skin picking and such are basal ganglia disorders and sometimes stimulants can improve some adhd symptoms but trigger problems in the basal ganglia. Intuniv is an adhd medication that can be used in combination but it helps the part in the frontal lobe that says "no do not do that" and it also helps your emotional control and it can reduce anxiety.
Let me guess the skin picking happens more often when you are distracted and mind wandering but also more often when your are emotionally exhausted.
Also CBT is one of the more effective ways to treat this disorder. While CBT is very useful sometimes you need a "crutch" to help you transfer one self destructive habit to another habit that is not self destructive before you can remove either habit. For example I have a spinner ring like this. but there are other ones avaliabile, like this one you can get with an engraving
Whenever I feel the need to move, especially when stressed I move the ring from my index finger and feel it around in my hands, but I also put it on my thumb, then I clap my hands together and use my other thumb to twirl the spinning part of the ring around. This way my hands are busy, but also I can't do anything with my hands unless I stop the spinning and unclasp my hands.
I do not really have anger management issues but other people in my family do. This is an anger management therapy as well for it allows you to channel energy and emotion into physical movement, prevents some bad physical movement, but also gives your brain more time to stop and think.
