Mine looks exactly like that. It supports VESA 75x75 and 100x100. The part you're showing easily slides onto the monitor arm.

Model is an A+ SMLC12. I couldn't find it outside my country, but you can Google it and get an idea of what would work. I'm sure Amazon would sell something similar under a different brand name.

Two separate mounts, I had to experiment a bit to get one that extends far enough out for the main one, and sits high enough up for the second one.
