Right now I am able to pretty much avoid sitting, but if I had to sit much I would get this to see how it helps. I went to see an orthopedic doctor, who did a physical exam and ordered an MRI. He was the first one to tell me ischial bursitis based on the physical exam, especially when my MRI came back normal. When the pain persisted, I also saw a physiatrist (pain doctor) who confirmed the diagnosis.
Generally, sitting as little as possible, icing (or heating), anti inflammatories (eg motrin), and using a special cushion when you do sit are the recommended first line treatments. I also find that regular walks and gentle piriformis muscle stretches have helped me. It’s also important to make sure any work or computer set up you have is ergonomic (chair not too hard, good back support, feet flat on ground, knees at ~90 degrees, elbows at ~90 degrees, top of monitor at eye level, wrist support for mouse + keyboard, etc.) - you can find a lot of resources about desk setup online.
The other thing that my doctor recommended if there was no improvement was a cortisone (steroid) shot directly into the affected area. I put this off for over a year because it scared me a lot. (My friend had a manic episode reaction to steroid injection and stole shit from his family.) I finally did get the shots earlier this year, and it was… bad. It didn’t help me at all. In fact, it made my pain significantly worse. Unfortunately, cortisone shots are pretty hit or miss. They completely relieve some people, others have to get them regularly because the effects wear off, and still others see no benefit at all. It’s worth discussing with primary care doctor or physiatrist and medical insurance company. Sometimes the cost is not covered by insurance (at least in the US).
The cushion I have is called the ‘socket seat’. It’s not perfect, but it generally does the trick - and I’m lost without it for any extended period of sitting. You can buy them online - I got mine on Amazon.
I got a special pad for my bottom - I found it very uncomfortable to sit upright.

I get it in my ass and the back of my thighs. I've been considering this one for the sitting bones cutouts.
I have a cheap one I bought at walmart quite some time ago. Nearly identical to this and beneath it I have one of these.
The addition of that beige padded seat cushion has helped a bit, but I still get a bit of pain. I think the sitting bones holes in the first one I linked will be more effective than the one with only the tailbone cutout. The purple ones seem good too, from reviews, but man the one I want is like 100 bucks lol.
