Right, can't find my copy IRL or on teh webz. However this book: http://www.amazon.com/Big-Wing-Paper-Gliders-Michael-Johnson/dp/0312067313 Has two of the gliders from it on the cover and the third looks like an evolution of another.
Can't guarantee it has that exact glider in it, but it will have others of similar caliber. It may also point you in the right direction to find a book that does contain it.
