I'm not. Most gamer gear isn't made with ergonomics in mind. I'm sure someone will eventually put one out, but I haven't seen one yet.
This weird thing is the closest I've seen. It appears to be programmable, so might be worth looking at. A far cry from a full numpad at your thumb though.
I just got this https://www.amazon.com/dp/B07THFMNQ2 but I DO NOT RECOMMEND. I happened upon this thread trying to find a way to fix the mouse rotation. The sensor isn't aligned with the direction it feels natural to hold and move the mouse in, so it trends up when going right, down when going left. After a day of using it my muscle memory is adjusting, but now every other mouse I have feels wrong because my arm is moving diagonally to adjust for the sensor positioning.
It's a damn shame, cause the shape is really nice and the clicks all feel solid. Unfortunately the joystick on top is just a 4 way hat switch. Doesn't even do diagonals so the default wasd binding is pointless. Having 5 shortcuts (stick is also a button) is pretty cool. Definitely way more suited for MMO gaming at a whopping 131g.

On a serious note: I have a regular vertical mouse, I played for about 30 hours on the weekend and I have 0 wrist pain. Vertical mice are great for a lot of gaming.
There might be some stuff on display at the local Best Buys or Office Depots, those sort of places, but I'm not entirely sure. Beyond that, I'm not sure you'll have any luck outside of buying used off of Craigslist or similar.
So I would say try finding some suggestions that might work for you, read up, and buy whatever you decide on off of Amazon. If you just plain don't like it, Amazon's usually good about letting you return it still for a refund so long as you don't abuse their good customer service.
My own flimsy recommendation would be something from Logitech. Their new stuff is expensive, definitely overpriced to some degree, but the quality is good. I like their shit. Again, whatever you decide on, get it off Amazon.
Beyond that, it sounds like you don't want to pronate your hand so you might also like a vertical mouse? The only "name brand" one I know of is Logitech's MX Vertical and I have no idea why it's so expensive. Amazon has a bunch of much cheaper, off brand ones. I found this "gaming" one by TRELC that looks pretty cool though; you might like it?
The only vertical mice on the market that are suitable for gaming are the ones that use the 3325 sensor.
You can try them, they are usually advertised with "10000 DPI".

These should be available in different forms in most of the world, but maybe with different names.
Recently there have been vertical mice with PMW3325 sensors on the market. (Should be plenty of IPS for that style of mouse)
You might be able to map the directions of the joystick to macros (don't know for sure)

I started playing VALORANT with this thing because I couldn't find another vertical mouse which had a 1000Hz polling rate. It was way lighter than the evoluent mouse I was using:

However, that vertical mouse was small and terrible for work. On top of that, despite the improvements I made, the aim corrections for gaming were still inconsistent.
Eventually I started trying normal mice for the first time in a decade, but JUST for gaming. And the key for me was using a claw grip for less wrist tension.
My current setup is a wireless evoluent mouse for work, and a G Pro superlight for gaming. I pair this with (almost) daily hand and wrist exercises.
Here is the TL,DR I have pretty severe RSI and am trying to remain employed. In the past year I have picked up a large stable of mice and keyboards to combat this pain.
I find myself gravitating to a few more often than not.
1)I use a Zealots vertical gaming linkmouse the one with the joystick on top. Pretty cheap on amazon.
2a)An Elecom Deft Pro link finger track ball and an 2b) Elecom Realcon link hand held thumb track ball. These are a bit pricey.
3) For the traditional shape, I use the Venus Smartech link mmo gaming mouse. Usually very affordable on amazon.
4)And 3 days ago I picked up the MX Ergo Pluslink. About $99.usd everywhere it is sold.
If you have a computer place nearby or work with an occupational therapist perhaps ask them for help. There are joystick options, some ergonomic mice may fit your hand more easily. Lots of stuff with non-traditional shapes. I searched for joystick mice on amazon and came up with lots of different stuff.
