I'm going to suggest prayer book resources.

If you say "I feel like I'm going to hell for X" there are deep problems in your understanding of God and his approach to your life. God is not some unstoppable wrath machine that's looking for an excuse to smite you. There aren't divine tripwires. GOD WANTS YOU WITH HIM. Period.
I'd suggest you take a look at some prayer books, to get an idea of how Christians have prayed for hundreds of years. Perhaps the Anglican Book of Common Prayer:

Or this ecumenical prayer book:

Or since I'm guessing you struggle with anxiety, maybe breath prayers would be of help to you. They certainly are to me.

(I also generally recommend anyone with anxiety issues take some vitamin D. It's a super-common deficiency that can make you feel like crap all the time for no obvious reason.)
