Yes you absolutely should start immediately. Your goal is to get your periods consistent and with ovulation. This takes time hence why people try to conceive for months. Okay it is this one and if you read the reviews it seems like it does EXTREMELY well for people who have PCOS or PCOS-like symptoms including irregular periods. Hey! So I actually ended up ovulating and getting pregnant in August of 2020 with no intervention! I was so irregular with my periods that the only reason I knew I was ovulating is because I was using the test strips to try to make some sense of things. what dancinghyrax said. Also, want to add that if you're trying to get pregnant with PCOS, I would HIGHLY suggest seeing an infertility specialist. They can check your uterine lining to make sure its within pregnancy range, as well as give you meds to help you ovulate even if you aren't having a period. If you aren't already, I would also HIGHLY suggest taking Inositol (40:1 blend) as loads of women swear they got preggers after taking it for a few weeks.

First cut out gluten. See how you’re doing. Then go a week no dairy. See if it helps more. And so on… If you don’t see a difference at least you tried.

Taking powder is hard for me, so I just take pills. I mainly take two a day instead of the recommended 4. I tried doing 2 in the morning and 2 at night yesterday, but I'm not sure if that made a difference.
I feel you! Do you have SAD? I have that, and by taking 5000 iu of Vitamin D per day, I noticed that my mood got much better almost immediately after taking the first dose. I used to take 250 iu in the multivitamin, but taking more is what helped. I also started taking Vitamin E. Since 95% of serotonin production is produced from your stomach, I've noticed that eating more nutrient dense food has helped! I also take a Probiotic that helps with GI issues and also take yogurt to help the microbiome that I almost ruined by eating too much processed food. I've noticed that I'm much happier in general, though I have a harder time to concentrate on things that I don't want to do. I'm focusing on getting Vitamin C from fruits + vegetables, and make sure to include food with high antioxidants like blueberries every day. I also take Hair, Nail, and Skin vitamins that initially helped to regrow hair, but my hair started falling off after going low carb, so I'm not sure how that will help.

I have been taking the Wholesome Story brand inositol supplement. I found it on Amazon. If you have any reservations about buying from Amazon they also have their own site where you can order. I have seen women argue that the brand Theralogix's Ovasitol formulation is the gold standard but it's considerably more expensive. Really you can choose whatever brand you'd like just so long as it is the 40:1 ratio of myo to d-chiro. This is the ratio with all the good results and has been most studied.

Look into Inositol. I am using this one in particular. Check out the reviews on it. It does amazingly to help lower testosterone and help get your ovulating regularly. Loads of women swear that it helped regulate their period.
My personal experience. I'm 36 years old and was told when I was 22 that I am infertile and will never have kids without tons of drugs because I do not ovulate. My periods without birth control are non-existent, I just don't have them. I was recently diagnosed with insulin resistance because of my weight but it was getting pretty bad to the point where I was throwing up every day. I hadn't had a period in over a year because I stopped taking my birth control.
I originally started taking inositol after I read great studies that showed it also helps with insulin resistance. I was desperate for help and didnt want to go on metformin because I was already dealing with bowel problems. It not only helped me immensely with my insulin resistance, to where I dont need to have a snack before bed so I don't wake up in the middle of the night to throw up, but for the first time I think I ovulated on my own because I had symptoms of ovulation.
This lead to 1 positive pregnancy test (I'm not TTC) and a negative blood test. But because I don't have periods, its a guess based on ovulation symptoms of where I could be at. So, I have had to wait 3 weeks before my doctor would re-test me, which is next week. Even if this is a chemical pregnancy, it's still a miracle in itself when I was told I would never ovulate without drugs to force me and that was only taking it for 2 weeks.
I've also noticed a reduction of facial hair, increase in sex drive, and increase in cognitive function, (before I tested positive). So definitely look into it. It's a supplement, so you can't go wrong. Start out at half a dose first though to let your body get used to it, and then increase in a few weeks. (I'm still doing half a dose almost 2 months later, I don't think I need an increase right now) side effects were temporary and went away.

Hit and miss it seems. I have IR as well and I just flat refuse to go on metformin unless as a last resort. Some people to absolutely fantastic on it, no issues, but I already struggle with digestive problems and my liver is that great... so I really just don't want to run the risk of side effects.

There are tons of posts on this sub that talk about inositol if you do a search in the search bar and that's the main reason I started to look into it to begin with.

Like you, I just can't do any form of birth control, I've tried them all and they make me feel angry and crazy. I also have insulin resistance, high androgens, IBS and a few other things but my biggest concern lately has been my insulin resistance and high androgens and so I started looking into alternatives for metformin (because I have IBS, I didn't want to make it worse with side effects from metformin). I just wanted to share because if you're like me, I'd rather try a supplement, than be on medication. I just tend to do better.

Okay so since it seems like you’re still getting your periods but you think you’re not ovulating, I would recommend Myo-Inositol with D Chiro Inositol. It’s a supplement that has been known to restore (induce) ovulation in women with PCOS and has also been known to regular cycles.

I recently went through almost everything you did. If you would like peace of mind, then get a fasting blood sugar test, as this is the one that can tell whether you are prediabetic. The A1C test is good, but the fasting test is better at determining insulin resistance (according to my doctor, my fertility specialist, and all the research I have seen). My fertility specialist ordered the A1C because of my PCOS, but no one ever ordered the fasting blood sugar test, even though they encouraged me to get it done. I'm not sure why, but I also would have had to request it from my GP.
As for what you can do: I am going to give you my background first, but my lifestyle changes are in the next paragraph. They have worked wonders for me, but everyone is different -- you have to figure out what works for you. My doctors have backed my decisions -- after the fact. Prior to my personal changes, none of them gave me practical advice beyond going back on birth control or going through IVF and then going back on BC.
I have lean PCOS. Went on birth control at 16 because I still hadn't had a period and my mom has PCOS (doctor rightfully assumed I did, too). I essentially got off BC at 35 when my husband and I started TTC. Acne immediately came back and my hair started thinning. After four months, I started to experience pain around ovulation and my period. The ovulation pain got worse every month until I ended up in the emergency room and taking off work each month due to the constant stabbing pain that would last over a week. Every doctor just told me to alternate acetaminophen and NSAIDs. This did absolutely nothing to curb the pain, and I felt like I was just poisoning myself. They all recommended that I go back on the pill, but we were trying, so I went to a fertility specialist to determine whether we could even have a baby. My husband and I agreed that I would go back on the pill if we had low chances, because the pain was ruining my life.
In the meantime, I decided to make a lifestyle change based on this group and my own research. I started a diet somewhere between Keto and Paleo: low carb intake (only raspberries, strawberries, blueberries, blackberries, or certain brands of dark chocolate), no processed sugar, and cooking with only whole foods. I also cut caffeine and alcohol for a few weeks. I purchased Inositol and turmeric and started taking it in the morning and at night. (I started with the Amazon brand to see if it would work before committing to the more expensive options.) In addition to this, I also got strict about 12-hour intermittent fasting (no food 7PM-7AM).
The results were instantaneous. It was very obvious that I was dealing with some serious internal inflammation. My pain stopped and has not come back, outside of normal period cramping. Imagine a year of almost constant pain that just stopped. I lost 10 pounds. I don't need to lose weight since I was already at a healthy BMI, but I look and feel even healthier now. I have since gotten my hair cut and my stylist said that I have fresh growth. My hair looks better. I still have acne, but I feel like that will never change unless I go back on the pill.
I have since started having a drink here and there, and I drink a little caffeine in the morning. I have switched to taking Inositol at night since some think caffeine impacts its absorption. Any doctor will tell you that there is not much research backing Inositol's efficacy, but that there is no any harm in taking it as a supplement.
My father died of pancreatic cancer, and in my quest to resolve my own issues, I constantly see information that makes me think about his diet and all the warning signs that he was dealing with issues related to his pancreas and diabetes. It is all so intertwined, and I just wish I had learned all of these things years ago.

I was reading other posts in this thread and they said I should be taking double what the bottle says (which I am not), so I can't tell you if doing that does any good. What I CAN tell you is that calorie deficit, walking daily, and this was my regimen for about 8 weeks before I got pregnant. I believe it's the reason I was successful, however, could be coincidence, could be caused by me actually taking care of myself. :) No idea!
