On ties, I use these toe spacers that you can find at Walgreens. You have to poke a hole through middle to put the mic through but they work like a charm. Really separates the mic from the tie material.
I think she buys them from her local pharmacy, but you can find similar versions on Amazon if you search "gel toe spacers". She uses the ones that look like this:

She recommended a simple in between tor 1 and 2. No wrap around either toe. I ordered these off of Amazon (begrudgingly lol I do not like Amazon)

I think I understand what you're saying - basically you have a extra space between your big toe and the rest so a tight shoe will push the top of your big toe towards your other toes making it look diagonal? If yes, I have a similar spacing issue so I wear silicone toe spacers between my big toe and the others which stops the movement and the pain. That said, I only use spacers with my pointe shoes because pointe makes the problem worse for me. I'm not sure if these spacers would be too clunky to go inside of flats. I personally wear a theatricals brand slipper Style No: T2915 and while I don't love the shoe completely, it definitely doesn't put too much pressure on my big toes.
