I went through the same exact thing, went to a dermatologist told them everything and this is what I got.
200mg doxycycline a day until it subsides Oil free acne routine (Eucerine moisturizer, panoxyl)
Ps. They didn’t suggest it but it helped me get the process started but Nizarol anti dandruff shampoo but use it as a body wash.

yes it's a shampoo :) i don't struggle regularly with dandruff so i don't know about the 2 in 1, just that nizoral is the next step when milder stuff isn't working. good luck!

Although I haven't used this personally, it seems to have excellent reviews. I learned about from the skincare subreddit as its active ingredient - ketoconazole, addresses the fungus that causes dandruff, the same fungus responsible for fungal acne.

use it for a 5 days, dandruff will be gone for at least a couple months. If it comes back, use it again. It won't ruin your hair if you only do it for a week.
Agreed on fungal. Probably tinea versicolor. To treat, you can get ketoconazole shampoo, lather it onto the affected area, leave it for 5-10 min, and then rinse. Do this once a day, and if it is tinea, by day 3 you should notice the patches starting to clear up. You don't need a skin scrape before trying this -- I'd try the shampoo first, and only do a scrape if the shampoo doesn't help.

Also are you against microneedling? I think it's worthwhile to try out as I'm doing that once a week while using the niz a few times and using gdx thick hair shampoo + conditioner. I try to do ~ 5 minute scalp massages while in the shower too.
I can second this, I had a very flaky/dry scalp and I tried all types of different shampoos. I had what looked like white spots/blotches on the back of my head that were visible due to my short hair, and the hairline at the top of my face would flake pretty bad. If you are balding already, then it will definitely accelerate when you jump on. If you are not balding, then the only way to find out is to do a cycle, but keep in mind that one cycle will not make you bald overnight, it will just accelerate the process.
I have a genetic predisposition to MPB and on 500mg test/week I lose plenty of hair. However, the following stack helps:
fin: 1mg/day
nizoral: three times a week
lipogaine: twice a day
You could replace fin with dut but educate yourself on DHT and the effects of lowering it aggressively.
Personally, I am happy with trading a few hairs for gainz, but it is up to you to decide which is more important; but remember, once cycle won't make you bald right away.

Ketoconazole shampoo 2-3 times a week. Go to the derm for full strength or visit the shampoo aisle at your pharmacy for the OTC variety. (In the US, it's labeled Nizoral.) On off-days I use Salicylic acid (T-Sal) shampoo. You can get the neutrogena stuff or you can buy the house brand and save some money.

The Keto shampoo doesn't dry out my hair at all. The T-Sal leaves it a little on the dry side, but I just throw in little conditioner. (T-gel left my hair feeling like straw. I hate it and don't use it.)
If you get particularly bad flare-ups around the scalp they derm can give a Ketoconazole gel or a stronger topical.
