I use the Oxford brand for notes and random scribbles. The Oxford Campus refill pad is a good option, it's got 300 sheets in it. Not quite college ruled, at 8 mm, but stil a great reasonably priced paper for fountain pens. It also has the Scribzee system, which lets you make a digital copy of your notes with the Scribzee app (which is free).
Depends on how large, and more especially, how wet your nib is. But, if you're in the US, look for Top Flight Wired notebooks with "Made in Vietnam" on the back - I prefer the ones with the wire at the top - out of the way of my writing. I can find them in my grocery stores.
Also, Walmart Exceed journals (soft and hard cover), should be cheaper than Kokuyo. They come in 78 and 100gsm - which you need will depend on your ink and the wetness of your nib.

I use the British brand Oxford notepads and notebooks for daily note-taking for school (I'm a teacher). It's fountain pen friendly (the optic paper). The Scribsee app makes the paper scanable, so you can make a digital backup with your smartphone. Don't know if this kind is sold in the US, though. Oxford campus refill pad
I'll add a slightly more obscure one that's fantastic with fountain pens: Oxford Campus A4 paper (Link to Amazon). Less than $6, 300 page pad (can buy the binder separately), but I've used all of my pens including the TWSBI Ecos I have: no bleedthrough. Zero.

For me, I would use the Maruman Mnemosyne A4 grid in landscape - with the spiral at the top. Here's why:
My second choice would be either Oxford Optik A4 (Amazon) or Black n' Red (Amazon, some office supply places). Kokuyo Campus notebooks might be OK, too.
If it must be picked up at the grocery store, Top Flight Wired notebook, as long as it says "Made in Vietnam" on it somewhere. Again, I prefer the top bound rather than side bound.
