I've always been told to start with a disturbance that the main character is dealing with. It doesn't have to be the main conflict of the book, but it's a hook to draw the reader in right away. A lot of readers, including myself, fall into the trap of sharing backstory (an info dump) in the first chapter to get the reader up to speed, but that's a mistake. Start with the main character dealing with a problem and taking specific action. Also, you want to somehow show your character in his/her normal world, dealing with his/her chronic internal issue that they will have to grapple with over the course of the book and transform by the end.
