I kept a diary with symptoms, medication, sleep and diet, which was just plain lined paper, but writing a full page was becoming too painful and I think the multiple choice aspect of a structured pain diary will help. I used to track pain levels in a spreadsheet too, but if you do that, make sure you have a guide as to what each number 1-10 means for its impact on your life.
