The best way to address your disorder is by going to a specialist that's trained in curing (social) anxiety.
Your medical insurance might limit what therapist is accessible for you. If so, go to your doctor and ask for a referral to a therapist specialized in anxiety disorders, or if the doctor has no access to that information, ask for a CBT therapist.

Give lots of hugs: Hugs release oxytocin, which improves your mood and relaxes you. So find people to hug. If you are single, hug your parents or friends. If you can't, see if a dog is an option. Most dogs love to hug. Another solution that provides the same benefit is a weighted blanket will provide a similar positive effect at night. You should try to aim for 12 hugs a day (if you currently don't hug a lot, I suggest you slowly build it up over time).

Yeah, I figured. You're not a severe case though (severe cases are people that are completely isolated, no partners either) and it might also be close, but not exactly right, some people with related issues will score high on this test as well. But that doesn't really matter for what we're trying to do, the advice should apply regardless. If you want a higher degree of certainty, a therapist can give you a real test (and that takes about an hour by comparison vs two minutes on this one).

In that case I can actually give you a bit more. I don't recommend people DIY their mental health (I just can't do that, since I'm just a dude that's good in using google, it wouldn't be right).
But at the same time if you do decide to go down that route I don't mind giving you what I have. From my own experience, it does take a considerable amount of time, don't underestimate that. In my own case (I'm therapy resistant, so I had to do it myself), definitely hundreds of hours.
Once it crosses the line towards a personality disorder, you need to study a lot, because there are nearly always self deceptions going on, you have to identify ways by which you fool yourself and that's a hard thing to learn. And the whole process is just one of discomfort.
All the ways you've been avoiding all those years have to be explored. But you do have the age for it to be able to do it with success. You understand enough about life and yourself.
Also, when you go in for that ADHD diagnosis, make sure you disclose properly, because ADHD medications will often increase anxiety (it's basically a simple axis, medication that treats depression and/or improves your ability to focus, also makes you focus more on things that cause anxiety, so they make your more anxious).
It's a game where you balance the two. You bump up your focus, often with medication and then you rapidly have to use that focus to learn how to properly manage your mind, so you reduce your anxiety. If you screw that up, you can end up using the focus the ADHD medication gives you to cause yourself anxiety.
And while therapy offers a lot of guidance with that, psychiatry doesn't give you that (at least in my case they never did, they do their best to avoid being confused with therapists, so I had to figure all of that out by myself). I think psychiatrists have some control over it, by trying to find a medication that works out more in your favor, but often that's a process of trial and error (and that again... Can be really uncomfortable, medication adjustments take several weeks, because you have to wait to see if the effects "settles" or not - meanwhile you won't be having a good time).

What you are writing in this thread is very typical for someone that's avoidant and the things you're writing to me point to severe social anxiety, which is also common with AvPD.
What I'll do is i'll give you information about AvPD, social anxiety and anxiety and tests are included.
Now, you're probably not going to do what you should do (because you're avoidant), I understand you're probably not going to do what you should do, but what we want is that you learn enough about yourself, so you can start working towards getting to the point where you can get that help.
It's important you take the time to read everything I give you. I understand it's a lot. But at the same time, you probably don't know these things (which is very common for AvPD).
Once you've read it, if you have questions, just let me know. But seriously... Start working towards getting help.
All this avoiding that you've been doing, it causes more and more anxiety over time. That is a cycle that needs to be broken out of. Until you do that, your life will just slowly become worse and worse and that's not something I wish on you.

The best way to address your disorder is by going to a specialist that's trained in curing (social) anxiety.
Your medical insurance might limit what therapist is accessible for you. If so, go to your doctor and ask for a referral to a therapist specialized in anxiety disorders, or if the doctor has no access to that information, ask for a CBT therapist.
You can also use this to search for a specialist in your area that is specialized in Social Anxiety, select Social Anxiety from the drop down menu. This will likely be someone that is specialized in CBT.
A small number of people that are treated will fall back in their old patterns and will have to go for a second round (which has a much higher success rate, so don't give up if that happens!). Those people also have the option to try medication in addition to therapy. Some people will respond better to medicine and some better to therapy, you won't know in which category you fall, unless you've tried both.

I don't mind giving you that information, but I do think it's time to get a therapist involved. By all means, start with self help, but while it's intended to keep you going (and it does so very well), it won't fix any of the underlying issues.
For underlying issues as well, there are ways to do this yourself, but it's takes a whole lot of studying before you can do something like that. Just for efficiency sake, if you can afford therapy, that's just way, way faster (because even best case we're often talking about a year of hard work already in cases like this).
I'll also add information about AvPD and a test for that. I suspect that is what this is as well (and who knows what else...).

You should try to aim for 12 hugs a day (if you currently don't hug a lot, I suggest you slowly build it up over time).

Give lots of hugs: Hugs release oxytocin, which improves your mood and relaxes you. So find people to hug. If you are single, hug your parents or friends. If you can't, see if a dog is an option. Most dogs love to hug. Another solution that provides the same benefit is a weighted blanket will provide a similar positive effect at night. You should try to aim for 12 hugs a day (if you currently don't hug a lot, I suggest you slowly build it up over time).

Let's see. So you're someone that deals with a lot of anxiety.
You also have very big problems with avoiding certain things.
Now the question becomes, have you been diagnosed or not? Because if you have been diagnosed, then you probably already know what's going on.
Just to speed up the discussion I'll assume you haven't been diagnosed (and then I assume that the reason for that is also that you've been avoiding that).
And then I know what's going on, most likely. But by now I've obviously been assuming a lot.
The problem is that if I'm right, you might avoid responding to me, because well, that might be scary too! So just in case I'll just keep going and who knows, I might get it all right and help you help yourself and save your life! (that would be awesome - and that does happen sometimes, by the way, not kidding)
Anyway, I might as well continue from here (because it's not a lot of hassle). I'm going to guess you are someone with AvPD, with severe anxiety and you deal with a lot of anxiety induced shame and guilt.
You also deal with with either [health anxiety](/r/HealthAnxiety) or BDD. That's just what happens if you walk around with untreated anxiety for too long, it's common.
And I think that's the most likely cause here. Either BDD or health anxiety. But just in case, go see a doctor, who knows.

There are reasons for that. First a therapist can't take "risks". If I guess what's going on and I'm wrong, you'll think I'm an idiot and move on. If your therapist guesses wrong, the bond between the two of you ends up damaged permanently. So I can jump to conclusions and hope they are right and if those conclusions are right, that you are ready to hear them.
Second, You're fully anonymous, so there's less risk on your end, and you are likely willing to show more of yourself. It's likely you are more guarded when you're in therapy.
Third, those two therapists probably made you think and gave you more self awareness and influenced your thoughts in this posts to some degree. In that sense, I'm a continuation on your road to recovery.
I'm not better than those therapists, we got lucky in many ways and hopefully you'll end up benefiting from that.
Now, it's possible you end up realizing you are therapy resistant at some point (2 therapist isn't enough to determine that, but you might conclude that at some point). That's not the end of it. At that point you can just train yourself up to the point that you know enough about what exactly is going on with you (and that will take a while) and then basically do therapy on yourself. It's not preferred, it takes a lot of effort (way more than therapy), but it is possible. So keep that in mind.
In a way you can see I'm already giving suggestions in that direction (as in, do it all, self help, therapy and medication). It's absurd how much effort is needed to fix deeper mental health issues. Takes a big human to do all of that. I think you either understand that now or you're close to understanding that. Maybe those therapists weren't good. Maybe you just weren't willing to sit down and take 18 months to build a proper trust bond (and for some that's what it takes and it's not unlikely are someone like that).
That's the really sucky part. Figuring that out. How can you know before you put in the effort if it will work? You can't. A good therapist, you will. If it's a crappy one, you just wasted 18 months and got nowhere. Beyond that the good news is that in general, with avoidant issues, the older you become, the less severe the symptoms for whatever reason. So you're rowing downriver, not up river. Might take a while to get where you want, but even if you stop peddling, you'll still float in the right direction, it will just take you longer to get there.

You should try to aim for 12 hugs a day (if you currently don't hug a lot, I suggest you slowly build it up over time).
Music: The right music can improve your mood. The genre is not important as long as it is: "Upbeat, rhytmic and energetic". One way to measure the effectiveness, of the songs is your ability to listen to it over and over (if you can listen to it hundreds of times it likely has the highest positive effect on your mood). But that is a big problem, you have to will yourself into someone that sees themselves as a person that is actively fighting their disease, that no longer identifies with it, or else you will unconsciously obstruct your own healing process. I think you have AvPD. Now, I'm not supposed to diagnose people and I'm not a doctor, but seriously I think you have AvPD. Everything you say is in line with it.
You have to improve to the point where you have the courage to go to therapy. Once you do, it'll take a few years to fix.
This isn't going to be fixed with medication. And you're not very mentally ill, it's just the AvPD. People with that always thrash themselves.
You have to get to the point where you start facing your issues. That's all. Baby steps. I know I'm giving you a lot to do and I know you believe there's no way you can do it, but seriously, you don't have an alternative.
That's no life that you have now and there's no need to walk around like that, most of it can be fixed without too much trouble, you are able to get yourself in the door.

That kind of does sound like avoidant issues. Look, you can't really "just" give someone a test for AvPD and then go "oh you got it!" for obvious reasons and that's not really what I'm trying to do. I'm trying to figure out "what other advice can I give you to get you going in the right direction."

That's closer to relationship insecurity (which is close to trust issues, but not exactly the same thing or at least, I view them as different).
Pretty bad. All things considered and you can't do therapy, I assume? Since you are self employed? Look, it's not impossible? I've dealt with worse (lol, he said with a deep voice while brooding over his glass of whiskey - no but seriously, I don't drink and I've dealt with worse). In fact it's starting to look like I'm also going to have to deal with several AvPD symptoms myself, in the near future, having lifted the narcissistic tendencies that were on top of it.
It comes down to some things you're already doing, expand it with things you're not doing yet. I'll add self help for depression and anxiety as well.
It basically comes to creating your own treatment program (which I had to do, because, well, there's no way I'm going to trust a therapist... Not a joke) and studying the various symptoms of your disorder.
Personality disorders are known to be hard to treat. Some people say it's impossible, but that's not true. It's just a matter of reducing the symptoms until you can claim to be "good enough for now". I'm not trying to tell you exactly how to do it. There's no set "order" with these things. Different people respond to altering symptoms in a different order and then there's a cascading effect "somewhere" that'll make addressing the next symptom easier. It's up to you to know enough of what's going on in general, to know what the various aspects are to work on.
And that's what I can do, give you the information needed, so you can know all that. Frankly, once you get started, it's kind of fun? Especially if you start seeing some progress (which can take a few months), but in the end, you are basically doing what a therapist gets paid big bucks for and you might even be good at it. The only limitation is that you're doing it on yourself.
So, AvPD, don't assume you "have it", might just be symptoms, but still it's likely that community and their resources can help you. Depression and anxiety advice, for sure should help you. Then I'm also adding social anxiety, because generally everyone with AvPD has that and there are some hints that you deal with social anxiety.

You really don't need more than 7 hours at most per night (a little more if you are under 18). If you can't fall sleep, try taking melatonin one hour before going to bed. If you find yourself awake at night, start counting. Don't grab your phone, don't do anything interesting. We're trying to bore you to sleep, not keep you entertained - sometimes it might feel like you've done it for hours and hours, but often it's really not long. Anytime your mind wanders away from the numbers, start over at 1. count at the speed of either your heartbeat or your breathing. The preferred way to do address your disorder is by going to a specialist that's trained in curing (social) anxiety.
Your medical insurance might limit what therapist is accessible for you. If so, go to your doctor and ask for a referral to a therapist specialized in anxiety disorders preferably, or if the doctor has not access to that information, ask for a CBT Therapist.
You can also use this link to search for a specialist in your area that is specialized in Social Anxiety, select Social Anxiety from the drop down menu. This will likely be someone that is specialized in CBT.
A small number of people that are treated will fall back in their old patterns and will have to go for a second round (which has a much higher success rate, so don't give up if that happens!). Those people also have the option to try medication instead, by going to a psychiatrists. Some people will respond better to medicine and some better to therapy, you won't know in which category you fall, unless you've tried both.
If you want to or have to solve this by yourself, if you put in enough effort, you will improve your symptoms. If you go this route, I suggest you set a deadline for yourself, then contact a specialist if you can't meet it.
How to resolve social anxiety yourself:
The best and quickest way to deal with anxiety, is to face your fear.
If you always avoid situations that scare you, you might stop doing things you want or need to do. You won't be able to test out whether the situation is always as bad as you expect, so you miss the chance to work out how to manage your fears and reduce your anxiety. Anxiety problems tend to increase if you get into this pattern. Exposing yourself to your fears can be an effective way of overcoming this anxiety.
The experience of anxiety involves nervous system arousal. If your nervous system is not aroused, you cannot experience anxiety. Understandably, but unfortunately, most people attempt to cope with feelings of anxiety by avoiding situations or objects that elicit the feelings. Avoidance, however, prevents your nervous system from habituating. Therefore, avoidance guarantees that the feared object or situation will remain novel, and hence arousing, and hence anxiety provoking. Moreover, avoidance tends to generalize over time. If you avoid the elevator at work, you will soon begin to avoid all elevators, and then all buildings that house elevators. Soon enough, you'll be living in a prison of avoidance.

I was under treatment for depression (and I did not respond at all to therapy, due to narcissistic elements in my personality, so I resist treatment) and then had the option to pay $100 for a diagnosis. You take a test and they then the therapist interprets the results. I don't know if that's easily to get from the NHS and I know from experience that most AvPDs aren't going to want to do that anyway, because they like to be invisible.

Well, now I have a better picture of you, I know you are not anywhere near my narcissistic side (in fact, you score is way, way too low, but more on that later) and lean more towards my schizotypal side.
I was referring to [splitting](https://en.wikipedia.org/wiki/Splitting_(psychology\)) (which if you do it, you won't outright know you do it, you do sort of hide it from yourself), which is typical among those leaning towards cluster B (and schizotypal isn't, it's cluster A and avoidant is C. And I don't know how common splitting is for them). You basically end up with a different kind of world, it's harder to understand and it takes longer to form.
Which might just not apply to you. You probably are dealing with similar issues, but it's based on that avoidant aspect (which I don't have, I don't avoid, in fact I will run straight at any fear that I can identify, I'm a fighter, I love beating or overcoming obstacles, including my weaknesses). With avoidants, they really fear being rejected, narcissistic people don't fear rejection, they fear being judged, being humiliated. If you reject me, but you do it in a way that doesn't lower my status (like publicly), I really don't care, but you will (rough lines, I'm not as narcissistic as I used to be, it doesn't matter).

Then I think you likely have low self esteem and probably social anxiety. The low self esteem I suspect because your narcissism score was just way, way too low. And almost everyone with AvPD has social anxiety. But I'll just give you the full information, that means there's also an additional test you can use to verify for yourself.
The schizotypal aspects, I just don't know too much about (yet). I know I have them, so I know it from experience, but I don't know how they play out in other people, so I don't really give advice for that yet. As I learn more, eventually I will, but I'm not there yet.

And I think that's the most likely candidate. Now granted, these tests aren't 100% accurate, but you're close to maxing out several of these scores. That's likely way above any margin of error.
Look, at some point, it is justified to just find a way and it seems like you have a job. This problem is likely going to follow you around and it will impact your life, quite seriously.
So I would just start saving for it. And if you have choice between spending it on something you like, but don't need or spending it on your mental health, I would spend it on your mental health.
And if you really don't have the income, I'd come up with ways to make extra money. And if you really can't come up with any, ask me and I'll give you some.
You can also try to make a deal, like instead of once a week, you go once every two weeks to therapy. That right there will cut your cost in two.

Your medical insurance might limit what therapist is accessible for you. If so, go to your doctor and ask for a referral to a therapist specialized in anxiety disorders preferably, or if the doctor has not access to that information, ask for a CBT Therapist.

Give lots of hugs: Hugs release oxytocin, which improves your mood and relaxes you. So find people to hug. If you are single, hug your parents or friends. If you can't, see if a dog is an option. Most dogs love to hug. Another solution that provides the same benefit is a weighted blanket will provide a similar positive effect at night. You should try to aim for 12 hugs a day (if you currently don't hug a lot, I suggest you slowly build it up over time).

People with high anxiety or OCD frequently ask for reassurance. This is negative behavior that should be stopped. The problem is that by seeking reassurance, you reinforce that you are unable to tolerate the discomfort of the uncertainty and you reinforce that you can fix this by asking.

Codependency involves sacrificing one's personal needs to try to meet the needs of others. Someone who is codependent has an extreme focus outside themselves. Their thoughts and actions revolve around other people, such as spouses or relatives.
Codependents often end up in abusive relationships, because they are specifically targeted by people that know exactly how easy it is to abuse codependents. Because of that codependent often have a horrible life, full of spousal abuse, either physically or emotionally or both. This really is not something you want to have happen to you, so it is very important you start taking action, before it's too late.
What causes codependency?
Codependency is usually rooted in childhood. Often, a child grows up in a home where their emotions are ignored or punished. This emotional neglect can give the child low self-esteem and shame.

Fundamentally, OCD is a very simple disorder, especially when compared to depression or self esteem issues. So it is easy to explain.
OCD is related to fear. Because you don't try for long enough to see what will happen if you stop giving in to the repetitive thought, your body never learns that nothing bad happens if you don't do what the thoughts tell you to do, so you will stay afraid of what will happen if you don't do what the thought tells you to do.
Start by understanding that the OCD thoughts are not yours (they aren't, because if they were your thoughts, you could control them).
