Codebasehq is always my goto. Had some great experiences with them and their customer support. It even does git if you want to switch over at any time. You are probably looking at about 10-20USD a month depending on what you do with them. Oh and their bandwidth is insane so lag is minimal.

The ability to review and annotate merge proposals, commits and (if possible) files inline. Github provides this on pull requests and commits, third-party tools provide that on patch files (e.g. rietveld).
