I use a program called "Free Alarm Clock" [1] and then I downloaded the audio of the video [2] using a site called YouTube MP3 [3] and set it to use that file as the alarm.
My computer is in sleep-mode, but the program wakes it up and then plays that file at a set time.

1st layer of protection: Download Free Alarm Clock and set it to wake up your computer from sleep, turn the volume up automatically, and play the most irritating sound ever.
2nd layer of protection: Hide your primary cell phone alarm somewhere unexpected.
3rd layer of protection: Hide a secondary, old cell phone under your bed for the last backup.
Set everything to go off in five minute intervals.
I've had to do this before.
I have one on my computer now so every time I want to turn it off I have to get out of bed, cross the room, type in my password (20 mixed case/type chars) and then either shut off the second alarm and go shower or go back to bed. It hasn't failed me yet. Of course this came after three years of snooze and skipping class followed by the three months before getting this alarm program of me shutting off my phone alarm without even waking up. So far the program hasn't fucked up my computer or done anything else besides be an alarm, so it may work for others. http://freealarmclocksoftware.com/
You're so right. And yes! Go for it. This is the one I use http://freealarmclocksoftware.com/ I pops up through the day when I set it and says things like "Talk a walk and hydrate! Choose life!" or "Take a walk and drink some tea!" or "Get up and walk! Choose life!" And I only had to set the alarms once. Now they just repeat every week on the days I scheduled.

I've used this for years, it's incredibly simple, and it meets 2 of your 3 requirements.
Pros:
You can copy it onto a USB and make it portable.
Can be used to play a variety of media.
Can wake your computer from sleep. (Turning on your computer would be pretty impossible).
Can turn power on to the monitor.
Shows a message on the screen when the alarm is triggered.
Cons:
I use this software on my computers so I can wake up to my favorite music and set a different track for each day. I imagine there's something similar for smartphones. I think the most important part of waking up on time is how you prepare for sleep. I would recommend F.lux if you do a lot of reading on your computer late at night. It's also helpful for preventing blinding screen syndrome if you're prone to checking your computer in the dead of night. If you have difficulty getting things done in the morning try and make a habit of preparing the night before. Try making cold-brewed coffee drinks. If for whatever reason you hate coffee and tea maybe take a caffeine pill in the morning. Try taking melatonin or eating tryptophan containing foods before bed. Get regular exercise during the day. Put on an eye mask before sleeping. Listen to white noise, soothing sounds, or recorded synchronized brain waves.
First things first... Download this: http://freealarmclocksoftware.com/
Next, set an alarm 15 minutes before you really want to wake up, choose alarm4 as the alarm you want to wake up to, and crank you speakers up as loud as they can go (if you have a subwoofer and high quality speakers, even better)... now go the FSCK to bed....
When you wake up, you may just shit brix, but you'll jump so far in the air / heart will be beating so fast, you'll forget all about sleep... by the time you shut the alarm off, you'll already be halfway dressed lol... works for me every day :-D
