I agree, it is mind-numbingly stupid to make "paste with format" the default.
One thing I like about OneNote is that it lets you choose which you want to use as the default.
Outside of OneNote, I use PureText - this binds Win-V to paste plain text in any program, although you can configure it to use any key combination.

This is useful if you are pasting from one document to another but don't want to copy over the old font. This is especially useful for copying in emails.
Every computer should have this.
I use PureText from http://stevemiller.net/PureText/. I've tried different programs and am very happy with this. Will paste unformatted text into any Windows application that accepts text paste. Hotkey is configurable: I use Ctrl-Shift-V too.

No one here that has heard of Pure Text? You can assign a custom key combination to this action. Ctrl + shift + v doesn't work in each application.
