One quick tip. You likely won't want a new record to get added to your core Customers table. Odds are you'll want to have a new Import Manager table that accepts data from the web. If you were to allow new records to be added to the Customers table, you would end up with duplicate records and spam. Updating existing Contacts or deleting spam is much easier done from a designated Import Manager table; where you can build some automated scripts that will identify duplicates and delete junk.
And while it is somewhat backwards, sometimes people opt to use a web-form service to collect the data. For example, Wufoo allows you to embed elegant and smart forms into any website. Once this data is collected, you use FileMaker's InsertFromURL[] script step to connect to the API and download the data.
One benefit of using a middle-man to collect the webforms, you'll never have downtime. Your solution, server, hosting, or internet will go to shit inevitably but if you use a 3rd party to collect the data, you'll have a 0% failure rate. Some clients don't care...if their cost to acquire a prospective customer is low. Clients that are spending $100s per inquiry will care.
What's the purpose in the password in the form? Are you using that just to prevent spam bots? Check out using Wufoo forms as an option so you don't have to do any server side scripts yourself.
I've used a few embedded form options but the last one I used was Wufoo because the client already had an account and was using it for other things. In this implementation I hid the form until you click the Request Item form and then the form is shown below.

Squarespace. It will give you a template, and you can drag and drop to customize the rest. You can handle the uploading of text to be translated with a custom form.
Do plan to spend a some time with the Squarespace documentation. It has excellent videos and written descriptions, but it will take you a few days to wrap your head around everything you will need to understand to get a site like yours built.

Hmm setting up our own automated recurring payment system with PayPal would probably not be easy/possible since there tend to be hoops to jump through for that. Maybe we can go through a third-party like Wufoo? You can set up a Wufoo form to set up a recurring payment system + also grab people's e-mails in the process so you can manage mailing lists for the big donors/reward tiers?
I don't know if setting up a manual system with PayPal where we all have to remember to send payments to you each month is gonna work out personally, but if you opt with that I'll jump on board. :P Maybe you could set up Google Calendar events we can join so we get reminders to pay?
