Another great app is "Lucidity".
It does all you mentioned, plus has a few extra features like a "dream sign analyzer" where it scans through all your entries and gives you repeated elements, allows you to attach audio/drawings to your entries, toggle tags, etc.
You could lucid dream multiple times a night but you would never remember if you don't have a dream journal which helps you with dream recall, trust me, tree months ago I was like "how could writing my dream help me remember them" but when I started doing it I was shocked that after about 1 month of either 1 dream or no dream recall I started to remember up to 3 dreams/night and if to u don't want to use a pen and paper you can use your phone to write down your dreams there is this app called Lucidity for Android
Download the lucidity app. It has push notifications and an ADA reminder and it reminds you in the morning to record your dream and at night to review that dream.

Not sure about the week question but I can tell you that if you can't recall dreams for a few days it's not gonna be a huge deal, it happens to many and it happend to me right before I had a lucid dream, I wasn't able to recall any dreams for about 2 days and then I just had a lucid dream without even trying and about the reality checks I like do do one every time I think about lucid dreaming or any of my dream signs, or every time I look at my phone but if you have problems remembering to do a reality check I recommend you Lucidity for Android, not sure what to recommend for IOS, also read the sidebar it will help

Without the use of an app, have a routine, like a certain sleep routine or periodically do reality checks (like pinching yourself).
Also keep a dream journal.

It's has a few nice tools to help you learn lucid dreaming and you can also simply use it as a dream journal.
I had the exact same problem when I started and the way I dealt with it is by saying to myself that every time I open a door I do a reality check and I also made myself a wallpaper for my phone to remind me of lucid dream also I recommend using Lucidity if you want an app to remind you to do reality checks, I promise it won't disappoint as the other app did I am guessing you used Awoken, I had problems with it too.Keep persistent and don't give up if you missed a few reality checks or forgot to write down your dream and forgot it, browse this subreddit everyday for motivation and keep trying!

I use Lucidity for Android. UI is pretty good. For me It's easier to type dreams with phone or tablet in bed.
