I used to use ballpoints every day for school. My hand would hurt during quick note-taking sessions. Since 6th grade, I've had a callous on my finger from the pressure I needed to get ink onto the page with a standard ballpoint. My parents refused to buy gel/rollerball pens since we have so many promotional ballpoints.
Now that I have been able to buy my own pens and ink (shoutout to QuickThoughts for the amazon gift cards) my callous is slowly fading away.
Yes, I still sometimes have to use the horrid peasant sticks for schoolwork as the copy paper is complete garbage, but FP's have helped my hand physically. I consider fountain pens not only as a hobby, but in a way, a medical treatment.

QuickThoughts (Android | iOS) surveys start at $2 and can be anywhere from 5 mins on up, and some will pay .10 cents if you disqualify, but they only pay out in Amazon and Apple/iTunes gift cards. I've had some decent success using them, but I haven't used them lately.
You could try The Opinion Room (just click-through the privacy error if you get it; they haven't renewed their HTTPS certificate yet), owned by Cint. They also have an app combining The Opinion Room and Panel Champ. Surveys are mediocre, and often don't pay the amount displayed -- sometimes more, sometimes less. I use them off and on and have cashed out a couple hundred over the years. They have $25 PayPal (less .50 for processing) as well as some gift cards.
Lots of people like Qmee -- I've seen people's posts in the monthly "Who paid you" threads with some decent amounts. I've used Qmee a little, but not as much as some.
I usually don't spend much time on survey sites since most of my money comes from Amazon Mechanical Turk, so I don't know much difference between high-paying and low-paying. But if I had to make some guesses I'd say Survey Junkie is very low-paying, QuickThoughts is fairly high-paying, and Qmee is about even. Just my opinion.
I'll give Forthright an honorable mention. I only do the surveys they email, and the nice thing is their loyalty bonus: for every 3rd survey they pay $2 whether you qualify or not. They have partner surveys, but they're the plain-jane surveys you find on other sites and the loyalty bonus doesn't apply to them.

You could also try the QuickThoughts app available for Android and iOS. It's a survey app that pays $2 minimum per survey, and they often last 10-20mins. Surveys are typical, but I seemed to qualify for more from them than I do from other sites/apps. It also sometimes gives you a .10¢ credit for dqs. Only 2 payment methods: iTunes and AGC. The minimum payout is $10. Must be a user for at least 24 or 48 hours (forgot which) before requesting payment. I had some mild success with it in the past.
Yeah, kinda sucks that SB banned you, but in case they uphold the ban it's nice to have a backup method or 2, especially if it's just AGCs you're looking for.
No unfortunately QuickThoughts doesn't have referral codes but if you look in the Play Store you'll find the app, same goes for the App Store for iPhones. The website is also easy to find. I hope this works out for you as well as it has for other users I've recommended it to :)
