Fast-paced twin-stick shooter with both single-player action against computer controlled opponents as well as realtime online multiplayer arena matches.

The Google Play content rating form does have some explanatory text, but interpreting can be subjective. I released a simple cartoon-y twin stick shooter (Tank Sector 4 if you want to see video) and got suspended for too mild of a rating.

I probably went further than necessary when tweaking the content rating because I didn't want a second strike. My IARC Generic rating went from 7+ to 16+.
If you follow the Handling Controller Actions and Supporting Controllers Across Android Versions guidelines your app should work with most controllers.
I made Tank Sector 4 to try controller handing and it has worked fine with a handful of controllers I've tried.
As for requiring controllers, you should probably have some kind of (bad) on-screen controls with a warning that the game is better with a physical controller. That lets someone at least get a feel for the game without hooking up a controller, and ought to tamp down inevitable 1-star reviews from players who don't respect the requirement.
