It's pretty detailed and was very helpful when I did my Annapurna Circuit trek solo. I think they have come up with a website as well where you can do bookings online by yourself, but not sure about the Annapurna Circuit area. Yeah, but the app is definitely recommended, super useful.
You could do the trek in 2 weeks but it's not enough for the entire Circuit. You can take a Jeep to Chame and start your trek there and end your trek at Jomsom if 2 weeks is what you are looking at.
Trekking till early December is considered fine. It's kinda perfect during mid November as very low number of internal tourist will trek after the holidays. And as far as avalanche, I don't think anyone should be worried till late December. And if you want to know about the weather situation, you can call the various tea houses especially at Deurali or MBC and get the info.
For the contact number, There's this app called "Trekking in Nepal" with all the details for free.

Enjoy the trek. 馃
I went there in August so I didn't book hotels. Just got them as we reached the place since it wasn't a busy time of the year. You might have to book at this time of the year since this is the peak time with lots of trekkers. Just in case. This app helped a lot for me back then: https://play.google.com/store/apps/details?id=com.trekkinginnepal.app (there's an iOS version as well)
It's got all the lodges with phone numbers and the app itself is very good and detailed... Should be of help for your Tilicho trek as well. And it's free now (I had to pay for it back then).
