Does this not work? I found it by entering "Fidelity two factor authentication" into google. The app was mentioned in the first link. That or Authy might be able to help with your other 2FA issues.
I had an issue with DTMF about a year ago, but it was only once and hasn't happened since.
You're only on wifi when you're at home? Those brick and mortar stores you mentioned probably have free wifi available. I usually don't bother connecting to them (and yet my phone continues to work), but they're there.
And on a personal note, my tablet has Android 5.1 and I hate it. Maybe if it hadn't rendered my IR port useless I'd have a different opinion, but as of now I'm planning on rooting it and going back to v4.4. It may be two whole versions behind, but at least it works.
You can use the ~~Verisign~~ Symantec VIP 2FA app, on Android at least. I don't know how it stacks up in security against Google's or Authy, but it seems good.

If this is about login, use a "security key" for Paypal by installing Symantec VIP Access. That will eliminate SMS dependency. I would still be hesitant about storing a large amount of money in a Paypal account or linking a bank account as Paypal has often frozen funds for arbitrary reasons and you can still hijack an account by social engineering a customer support representative despite the use of two factor authentication.
Wait.... I had to install VIP Access on my personal phone for getting 2FA prompt for the work vpn (Cisco AnyConnect). They can brick my phone using that? Is there any sort of phone monitoring possible via this app? (I don't connect to work VPN on my phone.)
Make sure to set up VIP Access on your phone first. You need to enter your Credential ID through the store's computer in order to login from home.
You can download VIP Access and use this for authentication instead of text messages. But the option is kind of buried in the paypal site.
