I use a productivity app and also combine it with the Pomodoro technique. I find myself more productive when working within the 25 minutes interval. Productivity app helps me plan and organize my work. Currently, I'm using Quire and so far so good.
This wasn’t my first go round with therapy by any means. I’ve used it on and off for the past 12-13 years due to varying mental health issues, primarily PTSD and anxiety. So I would agree with you that access to therapy was a very useful thing for me, but all of these changes were things that I had been struggling to make happen for many years prior to treatment for ADHD. The meds were the missing link for me :)
The thing with therapy is that different techniques work for different people. I find CBT highly effective. I like that I can come back to it without a psychologist if I catch myself spiralling, and with a good workbook (I like Change Your Thinking by Dr Sarah Edelman ) and honest self reflection I can pull myself out most of the time - or at least identify what the problem actually is.
It sounds like it’s been a hard couple of years for you, with such demanding work and your partner being sick? That must be exhausting. I hope you do find something that helps, therapy or a way to lessen your mental load or even a break!
