Word to the wise, try to get a better-than-a-dust cover like one of these sakosh bags, though this current lineup is out of stock and you are probably better off making your own. Also, TRC recommends Renapur to buff up this kind of thing

You get the leather cover, 3 inserts, craft card, see through zipper pouch, pen holder, pen, brass clip. Four bands lead to the inserts not sticking out. The quality of the leather is superb.

Yes, I was referring to the 016 pen holder <M>. I didn't realize that the other size (015 <S>) seems to be discontinued. The 015 only fits around the clip of a pen.
I should also note that the 016 pen holder pictured in my original post was purchased before the company was renamed from Midori to Traveler's Company. I have no reason to believe that the Traveler's Company 016 pen holder is different in any way other than branding, but I am including this Amazon link to the exact model I am using for reference.
One last note at the risk of repeating myself - The fit will be inconveniently tight at first. My initial thought was that I would need to settle for leaving the cap inside the loop permanently and just unscrew the pen as needed. However, with repeated use, it becomes much easier to push the entire pen cap-first through the loop like a syringe.

I can't bring myself to spend the money on the more popular leather covers like CS and Foxy Fix, so my "upgrades" are in utility and aesthetics rather than quality. My current favorite is my Webster's Pages TN cover in Diamond White. (Amazon link) Pockets in the front and back, and wide enough for those who like having more than a few inserts. It comes punched with two holes on each end, strung with 4 elastics. I improved it - added a third hole to each end and then mounted grommets on the holes, and then strung it with six elastics. I'm actually surprised by how much I love this thing.
I also have this leather cover, which is really good quality for the price. Unfortunately, I messed up the leather and until I do something with that, it's out of commission. Currently, I'm considering Angelus leather paint as an option for salvaging it. I grommetized that one too. Ever since I got my little grommet kit, I just grommet anything I can. LOL
One area where I've definitely upgraded is my inserts. In addition to using Midori inserts for my weekly and monthly calendars (I used to just print my own), I've been getting Tomoe River inserts on Etsy. So far I've gotten them from Paper Penguin Co. and I've got in order in transit from GoodInkpressions. I've switched to using fountain pens almost exclusively, so good FP-friendly paper has become essential.

I ended up pulling my cards out of the notebook, and just using a rubber band as a minimalist wallet.
There is this on Amazon. I haven't tried it yet; looks like it could be as annoying as the plastic one, just in a different way.

Another option is to buy a cheaper Field Notes leather cover with card slots, purchase a 1/8" or 5/32" leather hole punch, and then just punch a few holes into the "spine" section to insert the elastics into.

This is the tool I use, although it was only $9 when I bought it a couple years ago. It's a fine product, but idk if I'd spend more than $10 for it again. There are similar products on amazon.
It works well. When utilized with a straight edge, it produces a very professional perforation. I can perforate an entire Midori brand refill in ~4 passes w/ moderate to heavy pressure. It's another step in preping my Midori, but I would not call it tedious or troublesome at all.
The only drawback I've experienced is that it may work too well, in that if I keep a perforated page in the notebook and turn past it, the page folds at the perforation line and not at the natural fold of the refill. I used to worry that this would eventually result in one of these pages unintentionally falling out, but nowadays I leave a small section of the pages' top and bottom unperforated to ensure they stay in. It hasn't really been a problem, and I imagine that if you use a tool with wider spaced cutting sections, it would be different.

I just ordered a ZLYC fauxdori on Amazon for $12.99 USD... I added some Midori inserts and accessories. Seemed like a good deal. They have dark green at that price, and there are some nice embossed ones for a bit more.
