I have the SAME PROBLEM!! Squalene also caused me itching and breakouts !! I recommend sticking to a gel instead of a lotion (or something without all the oils)

I'm a newbie to PD but what I quickly found works very well for me is Dermacalm [SOS Therapy] by Amélie Monnier! It immediately soothes (will burn if your PD is already burning but will eventually soothe), flakes almost disappear, and it really helps to clear the breakouts - plus it's a 5oz bottle! I really pack in on like a mask every night to keep everything at bay, tap it on in the morning and reapply in the afternoon. Just know that when you put a thick layer on and it dries, it looks like white dry skin but that's just the product, everything underneath smooths out! Hope it works for more folks :)
I would say it's pretty common! It has a fragrance in it (basalm) which can be very irritating. It also has petroleum which is oil based and can really irritate PD (PD doesn't like oils.)
I had a lot of luck with this. It has fragrance but it's much less irritating.
I'll sometimes just put it on around my nose in the morning with a little powder overtop as it does make your skin rather white. I find it helps flare ups in general

Probably not exactly what you're looking for but this has been a game changer for me. It's 21% zinc so it's very calming and it's oil free and good for sensitive skin. I wear it like a mask to sleep and apply it a few times throughout the day. My skin is actually starting to clear up. When I feel it starting to itch and flush I put this on and it calms it right down. It's the only lotion my skin will tolerate right now.
I have not used that treatment plan. But I have fought off several bouts of PD with the following:
Completely striped down skincare routine- gentle face wash No topicals other than Rx Azelaic Acid and Sebamed Clear aloe for moisturizer
Other products that have worked: aloe gel with colloidal silver (antimicrobial properties) and this moisturizer

Hypochlorus acid spray has made a big difference for me. Here's one
I'm Indo-Caribbean and darker skinned and the only sunscreen that my skin has been able to tolerate is the Roche-Posay Toleriane Double Repair Moisturizer with SPF. It is the only sunscreen that hasn't stung or irritated my skin. My PD was steroid-induced so I also haven't had a flare up since stopping my steroid cream but my skin is very sensitive now and the only things that seem to work is Cetephil and the Toleriane line (a few items, mainly the moisturizers).

I’ve been using a neutrogena zinc face sunscreen which is very gentle and doesn’t burn my eyes. I haven’t had a flare up with it. One complaint ppl have about it is that it’s very thick and white (common for zinc sunscreen) but I have very light skin so it rubs in just fine. Make sure to dab it around face THEN rub which helps it from being too white and goopy.

In the meantime, I saw on this sub someone post that to try Exederm. It helps me when I start having flair ups but mine isn’t as severe as yours.

My lips looked VERY similar to yours for about 3 years though not nearly as bad. Both corners of my mouth looked like they were perpetually recovering from cold sores. My lips were chronically chapped. NOTHING would soothe them, and I tried it ALL. Vegan chapstick helped with the discomfort but did nothing towards healing. I've been following this sub for months and FINALLY found my cure!
It's a cream called Demodex, and you can buy a small jar for $50 on Amazon. Expensive but soooo worth it. Demodex, the cream I'm referring to, are also the name for a type of skin mite. The Demodex cream is meant to be a treatment for the mites.
Are mites and PD related? Does one cause the other? I have no clue. I feel 100% confident that I had/have PD (as confirmed by my Derm for other symptoms I have not mentioned), but none of my PD treatment helped my lips.
I'm no dermatologist, and I don't know if this will work for you... But I feel your pain, and $50 is a lot to spend on a cream especially after what you've probably already spent. I just know that my lips looked very similar, and I hate to see what you're going through. I hope you find something that helps. Best of luck.

Very interesting. I'm doing my own research now, and from the jump, it looks like Ovante Demodex Cream is the ticket- just as you suggested. I just ordered the "plus" cream from Amazon for $50. I'm normally a little leery of ordering skincare from random online vendors, but this is sold by Ovante, the maker. Walmart has it cheaper, but their skincare is sketch- too many untrustworthy, shady vendors. I'm interested in the shampoo, too, but I'll wait and see how the cream does.
The alternative to the Ovante Demodex Cream appears to be prescription 1% ivermectin cream that some say is super expensive--one said it was $100 for a 1 month supply AFTER insurance.
