You didn't say what platform you're running but I've had great success with Fission on OS X and with MP3DirectCut on Windows. Both are supposed to edit and join files without reencoding.
I use Overcast, the podcast app.
Overcast has a nicer UI than any of specifically-for-audiobooks apps, but does all the things you’d want from an audiobook app: remembers location, supports chapters, can skip back and forward in small increments, and supports changing playback speed. (Overcast includes the best speed-up tech I’ve ever heard: smart speed. Doesn’t speed up the actual speaking, just removes gaps. But I digress!)
The easiest way to get audiobooks into Overcast is subscribing to premium (~$10/year), which gives you 2GB of storage space and you then upload audiobooks into Overcast via the web.
A more advanced way (and this works for any podcasts app on any platform, not just Overcast!) is using Podcast Generator. Install Podcast Generator on any web server and have your own podcast of audiobooks: upload books as new podcast episodes and they appear in Overcast (or any podcast app) in a neat list.
Two caveats: If your audiobook isn’t a single file you’ll need to stick the little files together. I use Fission on the Mac (which supports lossless editing and can treat each file as a chapter), but I’m sure you can find audio editors for other platforms too...
The other caveat: Both these options involve uploading and redownloading the audiobook from the ‘net, not just directly loading them to your iPhone. But I find the convenience worth it, my Internet access isn’t slow, and I’m not worried about the piracy aspect: Overcast’s uploads are private, and/or just keep your podcast feed to yourself!
