JitBit Helpdesk fits the bill for you. This can be self hosted on a windows box. It is a simple .NET application with SQL back end. We've never had to reboot the server because of problems and we've run it for 6 years. It has LDAP auth and can do SSOs via SAML as well and has a great built in KB. It is a single purpose product that isn't trying to be all things and just knocks it out of the park for a simple easy to use dedicated helpdesk. It has translations for 25 different languages and also supports an App. It has an API/SDK so you can integrate it with third party products and has a reporting interface that allows to track technician's stats. Feedback from customers is built in via satisfaction surveys and you it can also serve as a knowledge base.

Try Jitbit Helpdesk (I'm one of the co-founders). It has a nice two-way email integration. Most of our customers use it with Gmail. I think it's a nice fit for a small company – it's very moderately priced and not bloated with features like many others. It's worth checking out.
