There's a journaling app that does this with trackers, It's Journal It!, I use It daily.
Another option: KeepTrack is a more specific app to this purpose.
I used Diarium some months ago but I tested Journal It! and now It's my journaling app of choice. I strongly recommend It. It's powerful not only for journaling, but also planning with Google Calendar integration.
Maybe you could try a Journaling app like "Journal It!" using notes/entries connected to places in a way that when you acess the in-app map, you can select your next action or read the info you need at the moment. There's plenty of organizing options such as Journeys, Actions, Tags etc.

Journal it, it's my favorite cos there are many functions u can write about a specific activity that u do everyday and it will keep a history just of this activity. There's another function called History, for exemple u gonna make a travel u can write everything in History so it's easier to review when u want Tag function, u can tag places, or subjects or ppl it's the best journal in my opinion I already tested the others but I didn't like

I'm working on Journal it!. One of the app's goal is track everything while keeps them organized. I can see that the custom stats feature fit really well for the app though I don't think the app will have that feature in a near future.
Anyway, for goal tracking, it's definitely better than Diaro :)

Currently, you can only export/import data to native format but I also plan to support export to JSON in the future.
