As far as my profiles go I have loads but I am going to go over my favorites.
Material Design has a companion plugin named "Snackbar Plugin" and that deals more with bottom sheets, etc.
That Snackbar Plugin currently does have a "Switch Sheet" which does (and is laid out) exactly how you want it in the pic, for the most part. Here is a Switch Sheet I created for switching certain Tasker profiles on and off. You'll notice it also supports a FAB button and an input field if desired.
/u/nickm_27 is the dev, and to Nick - perhaps a "Switch Activity" in the Material Design plugin would be cool, along with the ability to have the spinner, etc.

If you don't have it already you'll need to first download the Snackbar Tasker Plugin then configure an Horizontal Grid Sheet
Great work! Making Tasker Scenes isn't always the easiest endeavor, so I'm sure this was a great experience if just got Tasker to delve into the Scene Editor.

I think the <strong>Snackbar Tasker Plugin</strong> plugin has a trial version.
In fact I think both plugins have a full set of features that's available for free, and then they have additional features that are unlocked by making a purchase/donation.
You should go ahead and download/install/test them since I think you'll find there are many features immediately available that don't require a purchase.
It's like a pop-up menu. It can have text and buttons and stuff on it. It's a nice way to keep things uncluttered.

/u/Ratchet_Guy some time ago created a nifty Snackbar Profile Switch Sheet task that you could clone and customize to fit your needs. It uses /u/nickm_27's Snackbar Tasker Plugin.
If you forget their names, perhaps some should be renamed to the names you expect to find? :) If you do this, be sure to do a Search within Tasker to catch all instances, as references might not otherwise be caught.
I use it to link to apps in this sub, for example Snackbar. Highlight, choose AutoShare, my task "Playstore Link", and a couple seconds later, the link is there.
Also use it to directly download Fanfic from Archive of Our Own, I share the link to AutoShare and it processes the content to find the .mobi or epub file.
It also executes intents, I use it for a dynamic LastPlayer variable. You know how Tasker doesn't allow variables for Media buttons or applications? AutoShare does.
You should take a look at what layouts the <strong>Snackbar Plugin</strong> and <strong>Material Design Plugin</strong> plugins can generate. They may have something close to what you're looking for already, and they're alot easier to graphically configure.

Currently AutoTools Dialog does have Progress Bars (take a look under Dialog Type).
Switches can be found in the <strong>Snackbar Tasker Plugin</strong> "Switch Sheet".
The ability for Multiple input fields in one dialog exists within the <strong>Material Design</strong> plugin's "Input Activity".
Regarding Checkboxes - Currently I don't think there's a plugin that has checkboxes yet. So just Tasker Scenes right now have those.
I think the Snackbar Plugin might be what you are looking for.
Maybe in combination with the Autoshortcut Plugin, a Zoom- or KWGT-widget.
If you add a "Start Keep" Yes/No condition, e.g. with SnackBar plugin or AutoTools/Dialog, you could cancel the permanent start. Add a Cooldown Time to the profile - 10 secs should be enough.
But I have another idea that disables the profile for 1-60 seconds, but it is not predictable. If 61-120 seconds are not too long for you then this would be the solution. What do you think about that?

A somewhat workable solution for this would be to either use a plugin like the Snackbar Tasker Plugin or the AutoBubbles plugin ;)
Also wanted to mention you can also make the "Image Selection" dialog using the <strong>Snackbar</strong> plugin or the <strong>Material Design</strong> plugin as well.
Not sure if help you, but I've decided not use scenes in tasker anymore, was very frustating create one that was minimally beautiful. I have get this plugin to help my design flaws and concentrate only in make them useful.

The Snackbar plugin supports data entry fields in most of it's "sheets" Actions. So you've got several options there for sure.
The AutoTools plugin from the AutoApps Suite of plugins also has a Dialogs action which has several options as well.
Lastly there's Tasker's standard built-in "Variable Query" action which can take input, but it uses Scenes for it's look/design and isn't always the most visually appealling option.

For a simpler option, Nick is doing more and more write-ups on Material Design Plugin and it's been requested that a full app is created for people to copy.
For other simple snacks and such, I would also checkout by the same dev, Snackbar Tasker Plugin.

I think there is still a place for Scenes for very rudimentary designs and things like transparent blocking overlays, etc but for your purposes and simplicity's sake, I would look to Material Design Plugin as per above.
Definitely a great plugin!
For reference, when I set out to see if I could emulate a standard Android Material Design bottom sheet this is the original image I used. I assume it's from Google Maps.
Anyways using just this plugin, and all standard icons built-in to Tasker, I replicated the above menu/button and all. Here is a screenshot of the slide-up bottom sheet created with the Snackbar Plugin. Every item in that menu is fully functional just as it would be in any other Android bottom sheet.
The Snackbar plugin's new feature list is quite extensive at this point - Nick's put a lot of hard work into this. Just take a look at the Play Store screen demo images to see some of the awesome things it can do. Check it out :))

As far as how to display it without messing around with Scenes - I recommend one of two plugins pretty much made for this type of data display:

The AutoTools plugin from the AutoApps Suite of plugins, which includes the AutoTools Dialog Action to create all kinds of lists. This one is infinitely scrollable Here is a pic.
Just wanted to toss those out there for display options. I'll take a look at the data portion code and see if there's a alternative/different way to handle the data.
Well, it creates Snackbars, and Bottom Sheets, and Grid Sheets, and Switch Sheets, and more.
Have a look at the pictures on the Play Store for it as well as examples on the developer's website. They show some good example of the types of things it can do. The plugin basically makes it easy to create interactive elements that conform to Android design specifications, without having to muck around in the Tasker Scene editor to get the layout how you want it.
There's an excellent Snackbar plugin.
