No you don't but it will sync every download music on your phone though and since these watches have only about 2 or 3 GB of free storage it can't hold too much. Always pair your headphones to your phone if you have the choice.
Android Wear devices all have pretty small batteries
UI is nicer on the phone (and there's much more app selection)
However, if you want to have the freedom to leave your phone at home and go for a run or something else that you don't want your phone for, AFAIK right now you have two options:
Wear Music - this is $3, but its the only solution I've seen so far. This let's you transfer your own copies of music to your watch. The UI is nice and useful too. I just don't like the limited options on the card it provides, and the second card screen (when you swipe to the right) is just plain black with no controls.
For any android wear watch, you can sideload the sony music app to your phone, which will allow you to choose what tracks to transfer.

BUT none of these are very good for podcasts. Sure, you can play a podcast on the watch, but if you want to stop and pick up where you left off later (from the watch or your phone), you would have to manually fast forward to where you left off.

You could try downloading this app that acts the same as Play Music but let's you choose which music to download to your watch and then you can play it with the same app on your watch. Wear Music costs $3, but it does just that. On your phone you mark which songs you want to transfer. On the watch the app works well too (mostly...I emailed the dev with two problems I've been having and haven't heard anything back yet). Its easy to pick songs, and is really well designed IMO.
The issues I mentioned above are:
When you swipe right to the next card, the second slot is black on my SW3. In the Play Store, I think the last screenshot shows what is supposed to show (volume controls and next/prev track buttons)...but I just see a plain black screen that doesn't do anything when you press on it.
The next card after that black card is the "open" card which will open the app on the watch. The first time I press it, it will flash the app on the screen, but then go right back to the "open" button. The second time always works, but IRS just another quirk.
The best way to manage music is with Wear Music. It isn't free, but you get to select which songs sync from your phone (sorry, no computer syncing here).
Yeah I would try the headphones and see, maybe they don't have a good Bluetooth connection. That used to happen in my car and I switched the BT-A2DP adapter I was using and the problem was solved.

I have not tried it yet, but I don't think there's a music player other than Google Play Music that allows you to play music on the watch, so if the Google Play Music Wear app on the watch doesn't handle your own music files, then I doubt it.
