Check if the app can read your phone/SMS info. This can mean the app can read your IMEI, which is the only thing that would stay constant throughout all of those scenarios you listed.
If you have Xposed, you can try changing your IMEI. I would do a TWRP backup first, to be safe.

Hey! I just went through the same issue over the past month. The only issue I experienced is that the IMEI number was gone after I got that restored it has been working fine ever since. So if you have exposed use this app to put in the IMEI that your phone came with (it was on the original screen protector, and I think its on the box too idk though). If that doesn't work this guy has a video about how to restore its partition. (if you need an IMEI partition there is a guy, Rajneesh Gadge, who put a link to a backup of his partition in the comments). I learned most of this stuff going through my own struggle with the issue so I don't know too much about it but feel free to ask further questions if you need to
