It's an amazing piece of software, that you can import your games into and analyze deeply with any engine. It has a tactics trainer, opening trainer, and many more. Highly reccommended!

This one has the empty chessboard diagram as well as 90 move annotation. It is available on Amazon for $5.99USD. I gotta be honest though I just made my own notebook using a college ruled composition book. On each page I started at the red margin line and measured off 2x 1 inch columns, then 1/4 inch column for move numbers, then 2 more one inch columns for a total of 60 move spaces plus space to add clock times if you want. You will end up with the last column at about 2 inches wide which I use for notes and/or FEN notation of the board at any point in the game.
Now, I admit that I haven't used this notebook in a OTB game yet but I have done some practice runs annotating online games and it seems to work just fine.
A "must read" for players of all levels. I am still learning from it today after 5 years of chess practice.
