Arousals occur when stimulus passes both the brainstem and thalamo-cortical gate to awaken the cortex - stimulus that only makes it to the brainstem is called an "autonomic arousal" and only involves cardiorespiratory activation without cortex activation/EEG arousal. Arousals can contribute to cortisol secretion.

This is very interesting, I've asked my contacts at "3rd opinion institution" about this.
There is a chance that I have a significant RDI though. My first sleep study had defective EEG electrodes, so no EEG was recorded. My second sleep study was without a previous "PAP cooling off period" which, according to the 3rd opinion folks, messed up the sleep study. They recommend 2 weeks no BiPAP before the sleep study. This is why I'm postponing my 3rd sleep study for a while, I can't afford 2 weeks downtime right now. I'm going to be absolutely wrecked during this time, maybe I'll write a diary for you peeps :P

The idea is that you extend the lower section (jaw) a few mm out which also pulls your tongue out. This one is a bit bulky but you do get used to it.
A slimmer version is called SnoreRX (I don't think you can buy it on Amazon) but it is more expensive.
Tape has also worked amazingly well for me in the past few days. I had been using a mouthguard, but clearly that wasn't doing its job. I'm now dreaming again and I haven't for some time.

Not always. RERA scoring is optional. If you tell us what metropolitan area you're closest to users here could recommend you to sleep specialists that treat/diagnose UARS.

In 2005 UARS was merged into OSA, so technically for medical/insurance purposes you'll be receiving a "sleep apnea" diagnosis. Whether or not someone will have OSA/UARS will depend on which hypopnea criteria is used, whether or not RERAs are scored, etc.

I got a similar thing with CPAP. I was prescribed a CPAP but still feel like garbage on it, and I've tried both a bilevel and ASV and feel better but not great.
