This new book, which only came out a few weeks back, might shed some light on it. I haven't read it yet - I'll probably get round to it over Christmas maybe.

As far as I know, Dave started using hard drugs somewhere around the World Violation tour but that's conjecture at best. I don't think anyone outside of him and his inner circle know for sure and, frankly, it's probably not our business. The band was known for a lot of hard drinking and partying on tour for many years at this point.
This is the best regarded biography of the band to my knowledge.
