Despite what others have said on here, stick with Metamucil- 2 tsp AM and PM with 8 oz of water, followed by another 8oz of water. Metamucil does make stool slightly larger but more importantly SOFTER. It seems contradictory but you want bulky soft stool which relaxes the sphincter.
You mention that you are “picky” and eat veggies and whole grains yet you never mention how much or more importantly how much WATER you drink daily? Please aim for 100 oz daily and this does NOT include coffee or soda, nor does it include the Metamucil you are drinking
Elevate your feet while having a BM, ~6-7 inches to simulate a squat which is better for evacuating
As far as ointments You need to get 2.5% hydrocortisone (prescription only). Your doctor should be doing telemedicine and can prescribe that over the phone, So COVID is no excuse to not get medical care.

Basically THD (which also have some other similar variations or names like HAL, HALO) is another less invasive way that doesn’t use cutting.
Then there are surgeries that are less painful than a mechanical cutting, e.g. laser, or ultrasound (which is said to be even more precise and less invasive than laser). They all show promise but the challenge is whether it’s easy to find someone who can do them.
Also CRH is technically a type of banding, so potentially when a doctor says RBL there is a chance that they meant CRH rather than traditional RBL.

You might have tried many/all of these things. Really needs a Dr visit. If you're getting muscle spasms as well diltiazem could be an option and this would help to control the more acute symptoms. You might also have internal feeding into the external hems so banding could also help resolve symptoms without a huge operation.
I'm sorry you're going through this. I'm finally rounding the corner after two weeks of pain. I shyed away from much of the medication I had because it brought on intense burning (lidocane, hyrdocortisone) and rectiv was no longer working for me (life saver first time around).

Wiping is pretty harsh. If you need to use the bathroom outside of your home, you should order a pack of generic portable bidet nozzles - I got a 2 pack on Amazon for 5 bucks. You pop them on a water or soda bottle with a standard sized opening, and they work like a bidet. I tested one of them out and it works just as well as a peri bottle.
Depending on how much it is you could just use sanitary pads. I also used these for a bit after my hemorrhoidectomy (https://www.amazon.co.uk/gp/product/B074VCQYJ9/ref=ppx_yo_dt_b_asin_title_o06_s00?ie=UTF8&psc=1). They worked pretty well.

I might suggest using an ointment and coat your whole anal canal, yes you will need a glove and some privacy and yes you need to be gentle and not force things. The key thing here is coat your anal canal, all of it.
I got to thinking that internal hemms are tissue that get aggravated and get pulled as stool is coming through and over time this causes the tissue to stretch and expand this leading to large hemms and more complications.

It looks like an external hemorrhoid and it sounds like it's irritated. Are you having pain or bleeding with BM? Are you straining at all to pass stool? If you are, you need to increase your fiber and water intake. You can do this through diet or adding a non-stimulative laxative like miralax into your diet and using a stool softener so it's not so hard and rough coming out. Another thing for irritation is to ice the area, or soak the area in warm water for 20mins a few times a day. Preparation H (or store brand) hemorrhoid suppositories can help with the pain as well. Also, when you have a BM try to avoid using toilet paper and use water instead. You can buy a portable bidet/perineal bottle on Amazon that'll allow you to wash the area without further irritating the hemorrhoid.
Hey man sorry you’re going through this hemorrhoids are some bloody cocksuckers. I’m by no means a hemorrhoid expert but here are some things that might help you get them under control.
Get on a high fiber diet and drink a shit ton of water as well as take a stool softener so your stool is as soft as possible and will aggravate the hemorrhoid as less as possible.
Might have to stop hitting the weights for a while. I know it sucks as I’m a lifter too but lifting is one of the main causes of hemorrhoids because of the straining. Lifting weights is a great stress reliever and goal achieving activity but trust me stopping for a bit until your hemorrhoids aren’t wreaking such havoc is better for your health at the moment. Do cardio instead as that even helps hemorrhoids.

Look into rubber band ligand. It costs a lot less than surgery and totally gets rid of the hemorrhoid (new ones can form but get rids of existing ones). I just got the procedure today actually. Mine are internal though and it sounds like you might have external hemorrhoids. I’m not sure if the treatment can be used for both. If yours are in fact internal I recommend you look into this procedure.

I'm an old dude, I use one proudly and I don't care who knows. My comfort/health is more important than what people think. If people look at me funny, I just ask them "If you got poo on your arm, would you be content with just wiping it off with some TP?" Colace and Metamucil to keep the stool soft and easy to pass. 4x4 non-woven gauze like these to tuck between the cheeks. I haven't been bleeding enough to saturate something like an adult diaper, but I do seep blood/mucus onto the gauze that I change every time I urinate. The non-woven kind is much softer and less abrasive than normal gauze pads. If you have a bathtub I find it is much better for the sitz bath than the kind that goes over the toilet, although some people that have a lot of pain while having a BM prefer to have the over the toilet kind so they can have a BM right in it to ease the pain. It also helps to have either a shower wand or squeeze bottle that you can shoot some warm water over the area, it really helps to cleanse and "scratch that itch" at times.
Other than that just have some tylenol/ibuprofen stocked, you want to take as little of the prescribed narcotic as possible to keep from getting backed up. Have a lot of fruits, veggies, eggs, yogurt, soups, soft foods available. I would stay away from the meats and sweets for the first few days or so. I hope it goes well for you both!
