Digital Detox , if you have an Android, this app is so much better than Forest. It really locks you out until the time runs out. Very configurable as it lets you pick apps that you can use and it works so well too. I actually haven't used my phone for 13 hrs because i can't open stuff.
Digital wellbeing on android is built in and it lets you restrict app usage too.
I suggest Digital Detox if you are constantly checking your phone and you can't stop. You can choose what applications you want to block and you have to pay with real money to unlock the phone before the end of the timer.
I loved Zen Mode as well and basically had the same limitations. I found an app that is called Digital Detox that basically fixes the shortcomings (Play Store Link)

It lets you whitelist certain apps that you can still use while your phone is locked. Like I have my music and podcast apps allowed, as well as my gym tracker so I can still use it at the gym without the distractions of the rest of my apps.

It lets you schedule the times in advance, I have it from 6:30am to 8:30am every morning so I don't just lie in bed endlessly scrolling.

And it fully blocks notifications and you can't bypass it with the gestures which were a couple of issues I had with Zen Mode, though they might have been fixed now (I'm on 6T)

I miss the nice zen screen that the OP version has but since I'm not looking at the phone that much during the times it doesn't matter too much.
I would like to recommend the app Digital Detox Challenge (Android) for your third point. It blocks access to your phone for a few hours / days. You can even schedule that.

It's an amazing app , it gives two minutes of emergency use , And you can choose how much time you want your phone to be closed, you can choose the option with which you can never reboot your phone or shutdown the app when you are detoxing, And it also provides you with some suggestions of things you can do while detoxing , like going for a walk , or reading a book ...etc
I've had decent luck with a variety of "digital detox" apps. I use my Android phone's "Digital Wellbeing" settings to block certain apps after a specified period of time, another app called
"Digital Detox" to block all apps during certain time periods. For my laptop, I use a browser extension called Leechblock. There a lot of similar apps out there, so you should be able to find similar tools if you have an iPhone.

This is the most powerful app i've ever found , baaki sab apps ko easily force stop ya koi external method se bypass kar chuka hu but ye wala almost impossible hai cheat karne.
Install Digital Detox app from PlayStore , it will block you from using your phone except for the apps you can allow like calls , payment apps etc. This will help you get rid of mobile addiction just like it helped me. My screentime used to be 8-9 hours now its like 3-4 hours.

Just install an app blocker app and let somebody else set the password for you. No Chance of cheating, no need to get yelled at, just the slight frustration of not getting to check insta after you pull your phone out for the 12th time in 5 minutes.

Also remember to write a schedule of things to do instead and don't completely turn it off for ages otherwise you will bounce back and it will get even worse. Start by writing a list of things to do and block your phone for an hour maybe twice or 3 times a day and slowly bring up the time is blocked...

Basically the get around the phone block option is an in app purchase. If you add a password to the Play Store purchase settings, it's going to also ask for a password before you purchase the unlock.
I use Digital Detox
It has a lot of flexibility and lets you whitelist some apps to still use in the block (for example I have my gym notes and music whitelisted so I can use them at the gym while still having everything else blocked). You can specify lockout periods from 5 minutes to a month and can schedule them in advance. Like I have every morning blocked from 6:30am-8:30am so I don't just mindlessly scroll through my phone in bed.

If you're using an Android you can use Digital Detox, it's quite great. I use it all the time when I've gotta study and I know I'm gonna turn to phone when I'm not feeling like studying anymore. With this, I force myself to go for a brisk walk or a short convo with someone. It helps relax me, clears my mind and I also don't end up wasting way more time.

Detox is difficult with a smartphone. What I do is use this Detox app to limit whatever I don't need, while keeping all finance stuff accessible. You may try that. Hope you have a more mindful life.
