I've tried several icon packs, but the only one that's stuck for me is Long Shadow. The look is clean and material, the amount of apps that have icons is vast, and the masking is top-notch for those that don't have icons. The devs for this icon pack are also amazing! It's updated more than once a month, and every time it's updated I feel like the few apps I have that were masked previously get full-blown icons.

Icons are mostly Long Shadow with a few customs. They're pulled with Iconzy and I'm using the images with touch animations as part of the panel.
Weather info on the left pane is Material Weather Komponent.
Everything else is custom in KLWP.
Base launcher is Nova Prime with the Google Now Companion.
Still to do:
I'm not good enough with Tasker to do this yet, but I'd like to have everything in the Status Bar disabled when the Home screen is active, but displayed when a full-screen app is open. Soon I guess.
The actual logo is taken from this post by /u/gartsman. All credit to him for doing the hardest part of this work.
For those of you who might check out the PSD - it's kinda messy, but doesn't have many layers anyway. I never take the time to clean up my PSDs :(
