Use Voice Choice to create a rule. Then you can create Direct Dial icons.
I use it all the time for my Google Voice number for work. I put all my work contacts in a Voice Choice group and then I can dial those people using the normal dialer.
This is a known bug in the latest DOK2 update to Android 5.1.1 for the Note 4. If you took a recent OTA update, that is when it started happening. Go into Settings/About Device and find the Baseband Version line: it will end with DOK2 if this is the issue.
My earlier note about this has links to a couple of discussions of the issue with some partial workarounds.
One of the workarounds involves some carefully timed manual intervention on each outgoing call. The other involves some special Google Voice setup using the Voice Choice 2.0 app. That works OK but has two bad side effects: it takes quite a bit longer to dial each call, and the call log in your dialer will show your Google Voice number for every outgoing call instead of the number you actually dialed.
We are all hoping for another update from Samsung/T-Mobile to fix this!

I have the same setup. It's great. Company supplied my cell phone but I didn't want to port my number to them so I ported it to google voice and forwarded it to my corporate. Do you use any other apps to help with the integration? Have you looked at Voice Choice?
Here's what you're looking for. I'm using it every single day. My business # is a Google Voice # and I only use one phone for personal/business calls. It really gets the job done.
I've been using google voice for my personal number on my work issued phone for years. It's been fantastic, and I've been able to maintain two separate phone identities with one device. Work needs not know my personal number and my personal contacts dont need my work number. I use Voice Choice to automagic select what number to use depending on who I'm calling.
