I've been using Wear Stand-up Alert for the past two years. Customizable and comes with its own complication.
Wear Stand Up Alert Shows you how long have you been sitting and alerts you to take a few steps from time to time. Highly customizable: set length of sitting, set minimum number of required steps, set office days and hours when it will alert you. You decide if you want it to follow your DND setting. You can set alert type: just a change in the complication image, vibration or notification. Set it up once and it does its work in the background. Your watch can help you live healthier!
I've spent the past weeks improving my app Wear Stand Up Alert. Thank you to all who took part in the beta testing!
CHANGELOG
WATCHFACE COMPLICATION
Your feedback is appreciated. The current additions are all based on suggestions from the community.
It's not exactly a timer, but I've been using Wear Stand-up Alert to keep me from sitting at the computer all day. You can set it so it reminds you every 15/30/45/60/75.. minutes if you haven't taken a set number of steps that you choose.
Wear Stand Up Alert is a little known but pretty useful app. It has a watch face complication, tracks my activity and tells me if I have been sitting for too long. I like its configurability being able to set tracking days and hours. Also I can set my own time and step limits.
Google checks your app if you mark the "wear app" category. They asked for a few modifications in my latest app before it was given the approval.
Wear Stand Up Alert. Shows me how long I've been sitting and when to take a few steps to stretch. Advantage over some of the built in ones is the ability to set office hours and adjust reminder frequency and required number of steps.

seems you need 3rd party app like the one on the link (for wear OS) u/jd18_ 's answer is INCORRECT
Wear Stand up alert reminds you to get up periodically (every 30-45-60 min configurable work days and work hours, doesn't remind if you are active)
Wear Stand Up Alert lets you set an hourly steps limit and notifies when you are short of it. Has watch face complication, highly customizable, it even has a Tasker plugin.
Wear Stand Up Alert Make sure to place the watch face complication, so the OS would not kill the inactivity monitoring. Since the version 3 update developed with the help of many beta testers now works well with the Fossil Gen 5.
There is Wear Stand Up Alert for stand up reminders. You can set hourly steps goal and office hours too. An optional watch face complication shows how long you have been sitting.
It has been working well for me. You can maybe ask the developer to add breathing reminders too.
Update after 2 hours: we still have 6 + 20 codes. Nobody posted any screenshots yet for free Wear Stand-up Alert
Wear Stand Up Alert - tells me when I sit too long. Fully customizable active hours and days, inactivity time (15min to 120min) and number of required steps. I like that it automatically removes its notification when I actually get up. Complication shows inactive minutes.
Moonie large image provider to set as watch face background, so it changes with the phase of the moon: Imgur (I use this watch face theme)

Something is not right. Android System and the like should not use so much battery.

35-40 hours on my 14 months old Huawei still running AW1.5. I hope 2.0 won't be much worse. Depends a lot on the software you are using. The key to good battery health is:

No problem with battery under M9E42C.
Could be the software you are using.
I am using:
Battery life with these as before, or better. I usually have well over 50% at the end of the day with tilt-to-wake, always-on, brightness: day=90% / evening=40% / night=dimmed ambient / theater mode while sleep tracking.
If you are using software/watchface other than these, it could be one of them.
I'm using this on my Hwatch. Check it out.
Great. I am sending you the code via PM.
The app will act differently whether you have Bubble Clouds installed on your phone or not:
